Position Summary

Supports the professional clinical staff by providing direct patient care in the home setting and performing duties as assigned by the Case Manager. Ensures all actions taken in carrying out responsibilities support patient centered care.

Position Responsibilities

Performs housekeeping duties under supervision of a Case Manager.

Performs procedures as defined by department competencies.

Assists patient while maintaining the environment in a clean and orderly manner

Assists in duties as requested to meet the day-to-day operations of the department.

Assures safe, therapeutic environment for patients, families, visitors, and staff.

Participates in staff development and orientation of new staff.

Sets priorities and demonstrates organizational skills.

Plans, shops, and prepares nutritious meals.

Performs other assigned duties.

Qualifications, Knowledge & Experience

Required Qualifications (Including any licensure, certification, education):

Minimum of 18 years of age

Possession of a current valid driver’s license and must maintain eligibility for coverage under Mary Greeley Medical Center’s insurance policy.

Mandator Reporter training within 90 days of hire and must maintain throughout employment.

Organizational Requirements:

Maintain stroke education per regulatory requirements.

Preferred Qualifications:

High School diploma or GED

Required Knowledge, Skills & Experience:

Skill in effective oral communication in English. Ability to explain procedures and give information. Ability to report abnormal conditions appropriately.
