Overview:

The Home Alliance Coordinator (HAC) is a live-in position that provides a positive & nurturing living environment for individuals experiencing developmental disabilities. The HAC manages an assisted living home, provides support in all activities of daily living, and ensures community inclusion opportunities are provided. The HAC supervises other staff working in the home, ensures home finances are used properly, and ensures that all paperwork and reports are complete, accurate and timely. The successful candidate is creative, fun-loving and compassionate.

The Home Alliance Coordinator works 16-hour days and has every other weekend off. The HAC and is provided private quarters in the assisted living home rent-free.

Job Duties:

1.Assists in the development of Individual Support Plans, interaction guidelines and Plan of Care; develops written implementation schedules to assist the individual in meeting the goals outlined in the Individual Support Plan, Personal Futures Plan and Plan of Care.

2.Implements support services as outlined in the Individual Support Plan, Personal Futures Plan and Plan of Care, including but not limited to, activities of daily living, day habilitation, employment, inclusionary activities and active learning.

3.Provides direct care as needed in various activities such as personal finances, shopping, meal preparation, and activities of daily living, which include bathing, toileting, dressing, housekeeping, laundry, etc.

4.Assure appropriate maintenance, cleanliness and safety of physical site and home vehicle.

Education:

- High School Diploma or GED equivalent required

- Bachelor Degree in a health-related field preferred

Experience:

-Two (2) years experience working with people who experience developmental disabilities or providing direct support care required

- 1 year supervisory, teaching, or mentoring experience preferred
