JOB SUMMARY

The Shared Home Alliance Coordinator (SHAC) is a live-in position that provides a positive & nurturing living environment for individuals experiencing developmental disabilities that is shared with another Shared Home Alliance Coordinator. The SHAC works with the individuals supported in the home and works with the team to assure all paperwork for the home is completed and provides leadership to the operations of an assisted living home. This position involves direct supervision of direct care staff, ensuring home finances are used properly, and completing reports and documentation.

The Shared Home Alliance Coordinator schedule is 3 ½ days on and 3 ½ days off. Shift begins at 12 pm on Wednesday and alternates, either ending at 10 pm on Saturday or 6am on Sunday. There is a second HAC that works the other 3 ½ days of the week. The SHAC is provided private quarters in the assisted living home during their scheduled shifts. These quarters are shared between both SHACs.

An ideal candidate would be an energetic individual who is also creative and enjoys crafting, cooking, baking and participating in community events. The SHAC will feel comfortable with consistent and proactive communication with families, care coordinators, and other professionals.

Agency and individual specific training will be provided as necessary. Additionally this position shares home management with another co-worker so collaboration is key to success. In addition to providing daily support to the home residents, the SHAC must work with the home residents to complete a variety of tasks such as yard work, shoveling the snow from driveways and decks, and keeping the vehicle clean. Daily and monthly paperwork for both the agency and Medicaid must be completed within tight guidelines.

Responsibilities

1. Assists in the development of the Plan of Care; develops written implementation schedules to assist the individual in meeting the goals outlined in the Plan of Care.

2. Implements support services as outlined in the Plan of Care, including but not limited to, activities of daily living, day habilitation, inclusionary activities and active learning.

3. Provides direct care as needed in various activities such as personal finances, shopping, meal preparation, and activities of daily living, which include bathing, toileting, dressing, housekeeping, laundry, etc.

4. Assures appropriate maintenance, cleanliness and safety of physical site and home vehicle.

Qualifications

Education

- High School Diploma or GED required

Experience

- Two (2) years experience working with people who experience developmental disabilities required

- 1 year of coaching, mentoring, or supervisory experience preferred.

Other

Ability to lift 35 lbs, or 1/3 of body weight, whichever is less, on a regular basis

Must be 21 years or older, per State of Alaska regulations

Possess a valid driver license and at least 1 year of driving experience (may include permit time)

Pass a criminal background check under state regulation
