The Mental Health Case Manager provides case management and community based services to individuals on their caseloads, their families, and/or therapeutic home providers. The Mental Health Case Manager works with the treatment recipient's team to develop treatment goals and objectives. Duties include crisis intervention, advocacy, assessment, admission and discharge planning, interdisciplinary teamwork, and acting as a community liaison. This position facilitates ongoing collaborative and interactive processes between the treatment recipient and his/her family/guardian and professional service/treatment providers and/or the natural supports within the community. The Mental Health Case Manager provides individual skill development and facilitates skill development groups.

Job Duties:

1. Provides overall coordination and communication of needs and services for each

treatment recipient on their caseload, which includes interactions with Agency staff,

community-based service providers, and family members/guardians.

2. Maintains and manages a caseload and meets productivity standards of billable hours per

week, as required by Executive Management.

3. Ensures that each treatment recipient has ongoing active treatment in accordance with the

clinical recommendation in the treatment plan.

4. Works directly with treatment recipients on skill development, using appropriate

treatment models and interventions. Coordinates treatment with Mental Health Associates

and facilitates skill development groups.

5. Ensures that each treatment recipient has been provided opportunities for mental health,

medical, dental, dietary and/or health assessments and follow up. Assists with referrals,

applications, and follow up to various government and community resources.

6. Assures appropriate attendance, facilitation, and preparation for all Treatment Team and

Support Team meetings according to Agency standards. Assists in the development of the

Treatment Plan and discharge planning.

Requirements:

Education

- Bachelor's degree in Health Science, Social Science, Psychology, Special Education, or

related field required, with emphasis of study in mental health preferred.

- Experience in lieu of education may be accepted on a two for one basis in appropriate fields.

Experience

- Two years of experience working in mental health or with children or adults with

challenging behaviors required, one year of which involves responsibility for case

management or service coordination required

Other

- Ability to lift 35 lbs, or 1/3 of body weight, whichever is less, from a squat-and-kneel position

- Possess a valid driver license & at least 1 year of driving experience (may include permit time)

- Provide driving record that meets agency guidelines

- Pass a criminal background check under state regulation
