Position Overview.and.nbsp;The Behavioral Health Clinician, Specialty Care, is a key part of the health center treatment team. The BHC, Specialty Care assists members of the broader treatment team (primary care, dental, behavioral health) to identify, triage and manage patients with medical and behavioral health problems within the primary care settingand.nbsp;.and.nbsp;The Behavioral Health Clinician, Specialty Care acts as a member of the SVT Health .and.amp; Wellness Integrative Healthcare Team by providing specialty psychotherapy services to individuals, families, and/or their communities. The position works collaboratively with patients and the health center s clinical team to develop and accomplish shared goals for the patient to achieve coordinated, high-quality care. This position provides ongoing guidance, support and education to other members of the SVTHW Healthcare Team as it pertains to ensuring effective, culturally-sensitive, and integrated primary care for patients and their families.Essential Duties and ResponsibilitiesSupports primary care behavioral health integration, including: same-day access for assessment and triage of patients behavioral health concern; behavioral treatment using evidence-based interventions; close partnership and consultation with medical, dental and behavioral health colleagues to appropriately assess and facilitate patient access to behavioral health care.Assist primary health care providers in recognizing and treating mental disorders and psychosocial problems.Work as team with providers to coordinate care for patient.Work with primary care team and behavioral health team to triage patients and accept referrals to long-term specialty care behavioral health as appropriate through delivery of psychotherapy, support skill training, psycho-education and patient education strategies.For referred health center patients, conduct comprehensive behavioral health assessments, diagnose conditions, and formulate behavioral health interventions and treatment plansand.nbsp;Teach patients, families, and staff care, prevention, and treatment enhancement techniques.Provide timely and succinct feedback regarding consultation findings and recommendations.Document in Electronic Health Records in a timely, accurate and clinically sound way as defined by SVTHW policy.Submit billing daily that is accurate and complete.Maintain strict confidentiality for all patient/partners and their families and SVTHW information.Abide by the policies and procedures, clinical guidelines and protocols established by SVT Health .and.amp; Wellness, and state and federal regulations for practice as a licensed practitioner.Follow federal, state, local, and funding guidelines and requirements (to
