PROJECT SUMMARY:

This is a multi-agency project between Family Promise of Juneau (FPJ) and Saint Vincent DePaul, in partnership with AWARE for clients who have presented themselves through the Coordinated Entry System. This program will follow the National Alliance to End Homelessness and Supportive Services for Veterans Families standards. The Homelessness Prevention and Rapid Rehousing program (HP&RRP) will work with landlords to help prevent evictions and provide short-term rental assistance and services to those without housing to obtain housing quickly, increase self-sufficiency, and stay housed.

The goals of this project are to: Create a network of reliable landlords that are willing to house individuals and families leaving homelessness. Assist individuals and families in moving into realistic private market rental units of their choosing. Provide goal-oriented case management for up to one year. Offer limited tapering financial support that provides housing stability, but fosters the client’s independence.

The objectives of this project are: Placement in permanent housing within 90 days of presenting to the program. Case management and tapering financial support for up to one year. 80% of clients will exit the HP&RRP into permanent housing. 85% of those who exit into permanent housing will stay in permanent housing for at least one year past exiting the program without episodes of homelessness.

POSITION DESCRIPTION:

This position will work with the supervision of the FPJ Executive Director to enact the Homelessness Prevention and Rapid Rehousing Project.

PRIMARY DUTIES AND RESPONSIBILITIES:

Help create policies and procedures for this program that adheres to the fidelity of the project charter.

Create marketing materials to potential participants and landlords.

Develop relationships with landlords to help mitigate evictions and increase the potential of rapidly rehousing participants.

Demonstrate consistent high-level performance in a manner that enhances the method of service delivery, which is driven by individual and family needs rather than by specific program goals.

Manage workflow and workload assignments.

Develop, monitor, and evaluate all program self-sufficiency plans, outputs, and outcomes to ensure goals are met.

Participate in a highly collaborative partnership, in which choices are identified as well as consequences and self-sufficiency plans are developed and monitored on a frequent basis. Contact with participants should occur no less than weekly based on acuity.

Track data for service recipients.

Process referrals from other service providers.

Develop and maintain a strong knowledge base of community resources.

Develop and maintain relationships with other self-sufficiency programming such as education, employment support, and health care.

Advocate for participants with services providers, agencies, and in the community.

Represent and interpret the mission and work of the agency to the community.

Identify and maintain a working knowledge of critical issues and trends in housing, self-sufficiency, and economic security issues for low and moderate-income individuals.

ESSENTIAL FUNCTIONS:

Establish highly effective relationships with guests, community agencies, and volunteers.

Assess client needs and respond based upon Individual Self-Sufficiency Plans.

Utilize a Strengths-Based Case Management model in working with vulnerable populations.

Maintain confidentiality of all clients. Ensure accuracy and attention to detail in record keeping.

POSITION REQUIREMENTS:

A passion for the mission of Family Promise.

Bachelor’s Degree in Social Service Field, Social Work preferred

Excellent interpersonal skills with the ability to communicate and respect people from a variety of social, economic, racial, and ethnic backgrounds

Experience working with program development preferred.

Ability to establish appropriate boundaries

Training in Trauma-Informed Care or ability to obtain.

Ability to work in a team setting

Ability to manage multiple tasks

Must have a valid driver’s license, reliable car, and vehicle insurance

PERFORMANCE EVALUATION MEASURES:

Maintain compliance of funding agreements and grant reporting

Timely and accurate completion of paperwork requirements

Monitored and appropriate staff productivity and effectiveness
