Purpose of Position: Provide medical nutrition therapy for patients with diabetes and other chronic diseases; Provide health education to individuals and groups with chronic care needs to assure positive outcomes. Uphold the organization s vision, mission, and corporate values. Demonstrate understanding of and compliance with organization s policies, procedures, code of conduct and work rules. Essential Functions: Provide medical nutrition therapy, develop and implement nutrition care plans for individuals and groups in outpatient and inpatient settings Work with communities and diabetes team members to set and achieve community based diabetes program outcomes and be able to document achievements Collaborate with communities to provide diabetes and prevention activities related to fitness and nutrition Participate in health education for diabetes and chronic disease prevention projects Assit with the monitoring and reporting to satisfy grant requirements Supervise lay educators, prevention program coordinators and temporary staff assigned to the CAMP program Perform other duties as assigned
