JOB SUMMARY: Provides comprehensive case management and client advocacy under the Supportive Services for Veteran Families Program.

JOB DUTIES:

1. Responsible for receiving referrals and engaging clients within 24 hours, through face to face visits or telephone contact. Completes documentation regarding all contacts and information regarding the case.

2. Responsible for outreach and engagement of clients through telephone calls, face to face visits, and ongoing communication with all members of the Supportive Services for Veteran Families Team.

3. Oversees the collection and review of all collateral information necessary to assess client eligibility for assistance under the Catholic Charities Veteran Services (CCVS) Program

4. Responsible for developing and implementing individual service plans, making community referrals, on-going reassessment of the service plans and follow up. Updates service plans on a quarterly basis.

5. Coordinates all services for each individual client, including advocacy, giving information about community resources, etc.

6. Provides accurate documentation of services, including identifying resources, writing progress notes, detailed summaries of all services and contracts and submits as required or outlined.

7. Promote ongoing commitment to excellence and continual quality improvement to ensure the highest quality of services to our staff and clients.

8. Performs other duties as assigned.

JOB REQUIREMENTS:

1. Education /Background: Minimum of a High School Diploma or Equivalent with 5 years of relevant work experience. Bachelor’s degree in Social work or related field plus one year experience in the field preferred. Veteran preferred. Bilingual (Spanish) preferred.

2. Job Knowledge: Must have knowledge of Family Centered Practice model, community resources, Case Management, assessment and service planning, and problem solving with multi-problem families. Must be an effective communicator with ability to work with individuals and/or families in crisis. Must be able to work under pressure; must be able to meet deadlines and set priorities; must be well organized and able to effectively manage multiple cases; must be able to work independently; must be able to work as part of a team; must be flexible and able to adapt to changing working conditions. Must be sensitive to cultural differences within the agency and community. Fluent in Microsoft Office Applications.

3. Working Conditions/Physical Demands: Must be able to stoop, bend, squat, sit on floor and ambulate over uneven floor surfaces including stairs and steps. Must be able to visually and auditory assess and interact with clients to ensure their safety and well-being. Requires considerable work outside of the office and substantial driving. May lift up to 30 pounds.

4. Other requirements:

a. Valid AZ driver’s license, reliable transportation and ability to show proof of registration/insurance.

b. Level One Fingerprint clearance or ability to obtain fingerprint clearance; must maintain clearance.

c. You must be 21 years of age or older to drive on behalf of Catholic Charities.

Hourly Rate $16.11 - $19.33 DOE.
