Position Summary:

Under the direction of the Assisted Living Manager, supervises and oversees the routine daily nursing services of the Wellness Nurses in accordance with established operational procedures the assisted living settings in accordance with the mission and values of Royal Oaks.

Major Duties and Responsibilities:

Responsible for entering new orders in a timely manner when receiving a resident or when a request for assistance is needed by fellow nurse, following policy and procedures.

Operates within the scope of practice of licensure per the Arizona State Board of Nursing.

Responsible for Service planning, including the assessment of needs, and preferences, identification, planning, implementation of effective assisted living interventions and the evaluation of interventions in order to meet the planned services the resident is to receive.

Maintains a positive and effective relationship with primary care providers, and all service providers, as well as responsible parties and the residents in order to develop and coordinate a Service Plan that meets the residents identified needs.

Participates in ongoing Service Plan development and management, attends service plan meetings and communicates effectively to the team and external providers in order to achieve the highest practical well-being and satisfaction of the resident and responsible party.

Understands the scope of practice of each nurse and companion, and effectively and safely delegates only those tasks that are within the scope of practice as applicable to the supervised staff member.

Addresses problems and conflicts in a professional and effective manner with supervised staff, allowing for opportunities of learning and growth, and facilitating such opportunities in a respectful manner.

Utilize effective problem-solving strategies as relates to resident needs and build on resident strengths when developing the service plan, ensuring that holistic needs are addressed and services are implemented that aids in maintaining the overall personhood and individuality of the resident.

Observe supervised staff, identify and report educational needs, and be a routine participant in the probationary and annual performance reviews of supervised staff.

Administer medication, treatments and biological to residents under Physician’s orders and in compliance with accepted pharmacy and Royal Oaks policy and procedures.

Accountability of the storing and distributing of medication must be done in a safe manner during the shift.

Follow facility policy and procedure for reporting and documenting any incidents or variances of standard operations.

Ensures that medications and supplies are received to maintain adequate levels according to established orders and procedures, notifying pharmacy service of any variance errors, or of medication not received immediately.

Documentation must be completed per assigned tasks and within policy and procedure. Progress notes are completed to support the ongoing services provided to residents.

Maintains computer competency. Correctly query, modify and create all information in the resident’s electronic record gathered during the shift. Maintaining of skills by applying knowledge in the clinical setting is expected.

Maintains the comfort, privacy, and dignity of each resident in the delivery of services. Interacts with residents in a manner that displays warmth and promotes a caring environment.

Interacts courteously and appropriately with residents, family members, visitors, and other employees under a variety of conditions and circumstances.

Full understanding of residents’ rights, including the right to be free of restraints and free of abuse. Is responsible for promptly reporting to the Assisted Living Manager incidents or evidence of resident abuse or a violation of residents’ rights.

Assists in maintaining a safe, neat, and clean environment. Reports all deficiencies to the appropriate person, such as equipment problems, need for repair or special needs of residents.

Makes every effort to protect the personal belongings of each resident, including eyeglasses, dentures, hearing aids, furnishings, jewelry, clothing, memorabilia, etc.

Ensures facility and department policies, procedures, and guidelines, as well as local and state regulations, are met and followed.

When necessary, lifts, moves, and transports residents using proper body mechanics or lifting devices.

Performs all job responsibilities in accordance with prescribed safety and infection control procedures, including thorough hand washing, use of disposable gloves and other personal protective equipment where indicated, and proper disposal of soiled materials.

Utilize appropriate techniques for effective assessment, planning, intervention, and evaluation of residents and their needs.

Stays informed of trade and legal developments relevant to the nursing profession. Upgrades skills and maintains knowledge of trade/industry advances and new technologies.

Attends and participates in supervisory training activities, seminars, and meetings.

Performs other duties and projects as assigned.

Knowledge and Skills:

-Thorough knowledge of professional nursing practices/theories, techniques and tactics.

-Excellent oral, written, presentation and interpersonal skills.

-Computer proficiency Microsoft Word/Excel, Outlook and Internet.

-Organized, able to multi-task, think creatively and adapt to different situations.

Education and Experience:

-Minimum education level for LPN: successful completion of AZ State Board of Nursing approved licensed practical nurse program

-Minimum education level for RN: Associate Degree in Nursing

-Preferably a four-year degree from an accredited college or university in nursing, public health, or related field.

-Preferably three to five years of progressive responsibility as a licensed or registered nurse in a health care environment; or a combination of education, training and experience that would ensure the effective execution of the duties of the job.

-Current Arizona State Board of Nursing Licensed Practical Nurse Licensure in good standing.

-Competency in the position maintained through continuing education.

-Current CPR/FA certification from American Heart Association, American Red Cross, or National Safety Council.

-Current Fingerprint clearance card or the application thereof within 20 days of hire.

Communication Skills:

-Clear speaking voice, must be fluent in English; bilingual would be a plus.

-Ability to speak, work and interface with all levels of co-workers, residents, guests and employees.

-Ability to write routine reports and correspondence.

-Excellent telephone and customer service etiquette.

-Supervises non-exempt staff.

-Is able to react successfully and calmly to any situation that may present itself with a resident, guest, co-worker, or employee.
