Case Manager Job Duties:
ï· Admits new guests by reviewing records and applications; conducting
orientations.
ï· Determines guests; requirements by completing intake interviews; determining
need for medical, psycho-social, and psychiatric evaluations;
reviewing evaluations, treatment objectives, and plans.
ï· Establishes treatment programs by setting schedules and routines; coordinating
services being provided; arranging resources, including transportation and
escort.
ï· Monitors cases by verifying guests' attendance; observing and evaluating
treatments and responses; advocating for needed services and entitlements;
obtaining additional resources; intervening in crises; providing personal support.
ï· Maintains guests records by reviewing case notes; logging events and progress.
ï· Communicates guests progress by conducting weekly interdisciplinary meetings
and evaluations; disseminating results and obstacles to therapeutic team and
family; identifying treatment influences.
ï· Prepares clients discharge by reviewing and amplifying discharge plans;
coordinating discharge and post-discharge requirements; orienting and training
family members; providing resources.
ï· Improves financial status by analyzing results; monitoring variances; identifying
trends; recommending actions to management.
ï· Meets budget by monitoring expenses; implementing cost-saving actions.
ï· Prepares reports by collecting, analyzing, and summarizing treatment and results
data and trends; compiling statistics.
ï· Updates job knowledge by participating in educational opportunities; reading
professional publications; maintaining personal networks; and balancing work
requirements with learning opportunities.
ï· Enhances organization reputation by accepting ownership for accomplishing new
and different requests; exploring opportunities to add value to job
accomplishments.
The Case Manager will be thoroughly committed to PASS' mission. Concrete
demonstrable experience and other qualifications include: Documentation Skills, Analyzing Information , Decision Making, Research Skills,Verbal Communication, Written Communication, People Skills, Resolving
Conflict, People Management, Integrity, Connecting with Children and Team Player.
ï· A Bachelor's Degree preferred but may be substituted for a minimum of four years
of experience working with this population.
ï· Ability to work with minimal supervision.
ï· Valid drivers license and reliable transportation; meeting and shelter activities
require driving.
Must be local to the area and familiar with resources
Must be willing to work three evening shifts (5-8 pm)
