Geriatric Care Management, also known as Aging Life Care, is a holistic approach to caring for aging adults who are facing ongoing health challenges. Geriatric Care Managers work with families to identify issues and implement solutions to ensure quality of life for their loved ones. To achieve this goal, it is critical that a team of Care Partners are involved in the process – these Care Partners include nurses, physicians, in home caregivers, and other professionals.
Alternatives in Home Care is a local Geriatric Care Management Agency. We have served the greater Tucson area for over a decade. We are currently actively seeking Care Partners to join our team and to assist in caring for our clients. Currently, we are in need of in home caregivers. In this role, you will assist clients in their home with Activities of Daily Living. You will work with the Care Manager to identify and implement effective care solutions to enhance quality of life and keep our clients safe in their homes. Scheduling is flexible, and you will be able to choose shifts that work with your schedule. However, it is CRITICAL that you fulfill your commitment once you accept a shift or series of shifts.
Experience as an in home or facility caregiver and the ability to work 24-hour shifts 3-4 days per week is highly desirable. Flexibility is also important, as client needs change regularly. In addition, you must demonstrate the ability to work with persons with dementia, have excellent communication skills, a valid drivers license and be discreet, professional and mature.
