STUDY PARTICIPANTS FOR CONSUMER TRIAL OF A NEW DIABETES GLUCOSE MONITORING DEVICE.

TESTING LOCATIONS:

YUMA, AZ - MONDAY March 18, 2019 (THIS IS THE ONLY DAY WE WILL BE IN YUMA)

We are seeking non-diabetics and a limited number of diabetics to participate in an evaluation on the usability and function of a NEW Blood Glucose Monitor.

Blood Glucose Monitors are used by diabetics and pre-diabetics on a daily basis to determine their blood glucose level. The results of these tests can help the diabetic or pre-diabetic determine proper dietary intake and/or medicine dosage.

Participants chosen will be required to perform a series of Blood Glucose Tests on themselves AND allow a Med Tech or other healthcare point-of-care professional to conduct a series of Blood Glucose Tests on the participant. Additionally, at the conclusion of the testing a finger-tip capillary blood collection will be performed on the participant by the Med Tech or other healthcare point-of-care professional. The capillary blood collected will be used for a Blood Glucose Level Confirmation Test conducted by a laboratory. (The confirmation test is used to determine the accuracy of the results obtained on the new device by the participants and by the professionals.) Participants will be required to prick themselves using a lancet device/lancet and they will be required to allow the healthcare professional to prick them using a lancet device/lancet. Specifically,

Participants will be required to:

Prick their finger-tip and perform a glucose test using the new device.

Prick the palm of their hand and perform a glucose test using the new device.

Prick their forearm and perform a glucose test using the new device.

Participants will then be required to allow our point-of-care professional to:

Prick the participants finger-tip and perform a glucose test using the new device.

Prick the participants palm of their hand and perform a glucose test using the new device.

Prick the participants forearm and perform a glucose test using the new device.

Note: Participants will be provided user manuals and instructional guides showing them how to use the lancet device, lancets and blood glucose meter. However, participants should understand that some discomfort may be felt when performing finger, palm and forearm lancet pricks.

At the conclusion of testing, Participants will then complete a short 2-3 page questionnaire describing their thoughts and views on the new device.

NOTE: This questionnaire will include the collection of certain demographic data to be used by the company to show the diversity and makeup of the study group.

Study participation will take approximately 30-45 minutes. (Depending on Ability May Take Longer)

Type 1, Type 2, Pre-Diabetics and Non-Diabetics are able to participate.

Participants will be compensated 50.00 at the conclusion of the testing and completion of the questionnaire.
