You can make a difference without having to do direct patient care. Due to tremendous growth, we are expanding with additional openings for Chronic Care Coordinators in our Fort Smith, AR location. As a member of our Chronic Care team, you will have the opportunity to help improve the quality of life for our patients by delivering one-on-one clinical guidance and support.

Together, we're removing the barriers that prevent the patient's from receiving the kind of quality health care they deserve in hopes of keeping them out of the Emergency Room. We focus on collaborating with Primary Care Physicians, Specialists, and Hospitals to create a comprehensive, individualized care plan around the patient's chronic conditions.

Job Description:

Your primary responsibility is to provide at least twenty minutes of care coordination for Medicare patients each month. The key tasks within your job will include telephone interactions with patients, documentation of patient interactions, care plan development, chart reviews, care coordination between providers and other facilities, and the ability to work well with the patients and providers to optimize patient care.

Position requires:

Strong computer literacy (Microsoft Windows, keyboarding skills, strong systems aptitude, etc.)

Strong computer & telephone multitasking skills including: the ability to effectively search for and type information on the computer, navigate through multiple windows and screens quickly, and inputting information accurately while keeping pace with the call.

Excellent verbal and written communication skills

Exceptional people skills with computer knowledge and the willingness to learn

Must be a self-starter with excellent communication skills and patient assessment skills.

Excellent interpersonal skills with patients, Primary Care Physicians, Specialists, and colleagues are a must.

Current Medical Assistant Certification or LPN License Required

Office hours are 7:00 a.m. to 7:00 p.m. Monday thru Friday. We offer flexible schedules.
