The worker is required to have close visual acuity to perform daily activities. May be subject to inside environmental conditions, hazards (including electrical currents and exposure to high heat and chemicals), infectious diseases, and may be required to function around prisoners and mental patients.Skills: Ability to identify scope of practice issues, prioritization, effective decision making, communication and interpersonal relationship skills. Appropriately applies knowledge, training and skills in the care of neonate, adolescent, adult, and older adult. Successfully completes BCLS Modules 1, 2, 3, & 4. Successfully completes SVHS Advance LPN Course within 6 months of start date.PRIMARY CUSTOMERS: patients, families, physicians, and co-workersREPORTING RELATIONSHIPS: RNs, Charge Nurse, Unit Manager, Supervisor, Director, and PhysiciansIncumbent has access to restrict or confidential patient or other highly sensitive business information and must comply with the terms of SVHS/CHI Policies as they apply to their job role.AGE GROUPS: Neonate/infant (Birth - 1 mo) Infant (1mo-1yr) Toddler (1yr-3 yrs) Preschool (3 yrs-6yrs) School Age (6 yrs-13yrs) Adolescent (13yrs-18 yrs) Adult (18 yrs-64 yrs) Older Adult (65+yrs)ESSENTIAL FUNCTIONS:

GENERAL CARE

Uses appropriate knowledge and skills in the care & interaction with customers across the age continuum.Performs care according to hospital / departmental policies & procedures.

COMMUNICATION

Identifies self and role to all customers.Ensures that all team members are aware of key information essential to patient care.Seeks & offers assistance as needed.Utilizes conflict resolution skillsCommunicates in a positive professional manner when relating to customers.

PATIENT EVALUATION

Gathers historical data and documents in admission notes.Evaluates patient’s need for medication prior to administration.Evaluates changes in patient condition and intervenes appropriately.Evaluates patient allergies and medication incompatibilities.Documents interventions in nurses’ notes.Identifies potential risk factors and intervenes appropriately.Conducts ongoing assessment using nursing process.

ADMINISTERS MEDICATION

Administers and documents medication.Evaluates and assesses patient response and documents appropriately.Accurately clarifies and verifies physician orders prior to administration.Correctly uses equipment (IV pumps, PCA/Epidural, etc.).

IMPLEMENTS THERAPIES AND PROCEDURES

Verifies orders [physician, nursing].Verifies presence of informed consent.Performs procedures according policy.Evaluates and documents patient’s response to procedures, therapies, and medications.

TEACHING

Assesses the patient’s and significant other’s knowledge base, ability and readiness to learn based on their developmental age.Schedules teaching for maximal learning.Presents education at learner’s level of understanding.Documents patient education in the patient care record.Completes self-learning goals each year.Shares learning experiences with others.Attends unit meeting / inservices and all mandatory inservices.Documents staff education per unit policy.Evaluates teaching results and adjusts teaching to maximize learner success.

MAINTAINS PATIENT RECORDS

Documents according to hospital policy and condition of patient.Follows plan of care/clinical pathway.Additional Information
