Check ER tracking shell for admission icon.

Collect medications lists from various sources.

Perform patient interview using defined list of probing questions.

Document current medication list in electronic medical record with high level of accuracy and completeness.

Identify issues with medication history and research discrepancies with alternative sources (e.g. physician’s office,

pharmacy, patient medication bottles, external Rx History in Cerner)

Communicate with nursing and other personnel regarding status of the completed medication history.

Must perform essential duties at high level of accuracy as routine audits for success will be performed.

Communicate with nursing and other personnel regarding distribution of medication; answer phone, refer calls, etc.

Assist in training new personnel in duties related to area of specialty.

Perform other related duties as assigned.

QualificationsMINIMUM QUALIFICATIONS

Education: Associates Degree/College Diploma and/or equivalent pharmacy experience.

Experience: Prior Med History Tech experience preferred. If no prior experience, must have at least two years of experience as a retail pharmacy technician or have been accepted to a four-year program of study leading to the Doctor of Pharmacy (Pharm.D.) degree.

Knowledge Base: Must score > 80% on the Medication History Technician Exam (given during interview).

Licensure: Must be registered as a Pharmacy Technician or Pharmacy Intern with the Arkansas State Board of Pharmacy.

Skills: Must possess excellent communication skills and be able to effectively communicate in an environment of high stress. Must possess ability to comprehend advanced computerized systems. Must be able to deal effectively with all types of people. Must possess proficient skills with Microsoft Word and Excel. Must possess good organizational skills and be able to function independently with minimal supervision. Must be able to input data quickly and accurately into relative computer software. Must possess good problem solving skills and high level follow through when obstacles in data collection occur.

Physical Requirements: Must be able to stand and sit for prolonged periods of time, up to 6 hours. Individuals in this position could possibly be exposed to blood borne pathogens and body fluids, chemical hazards, respiratory hazards and radiology hazards.

Leadership: Must lead by example by providing positive role model. Must be able to adapt and apply feedback from constructive criticism in a manner that is positive and professional. Must consistently demonstrate positive customer service skills and ability to actively participate in departmental and other work group projects as assigned. Must be able to assist with Pharmacy Tech duties in other areas of the pharmacy as needed.
