SUMMARY OF RESPONSIBILITY - Job Summary (general statement of purpose or objective of position)
Triage Nurse will be responsible for the navigation of health care needs of home health patients during the call center hours of operation. Navigation will include following care pathways, assessing patient needs, instructing patients on medication, wound care, scheduling a home visit the following morning, and/or escalating the patient needs to the on-call nurse for a home visit, directing to the nearest ER, etc., as warranted by the agreed upon processes and procedures. The Nurse will be required to access electronic medical records and document patient interaction specific to each facility including the Triage system. Triage Nurse will upon occasion be required to attend education and training at the BH Support Center.
MINIMUM REQUIREMENTS - Knowledge, skills, abilities, license, registration, certification, education, and experience
Minimum Requirements:
Registered nurse currently licensed to practice in the state of Arkansas required.
One year practicing as RN in an acute care hospital, clinic setting or equivalent
Requires good interpersonal and communication skills.
Unencumbered license
Preferred Skills:
Previous call triage or case management
Previous home care experience
Certified Bi-lingual is a plus (Spanish, Marshallese
