Nurse Care Manager -conducts home visits throughout Kern County

Education: Associate or Bachelor of Science degree in nursing

B. Experience: One year of experience in geriatrics or care management preferred.

C. Hold a valid California Department of Consumers Affairs Board of Registered Nurse license that is current and in good standing.

D. Hold a valid current CPR/BCLS card.

E. Special Knowledge, Skills, Abilities: Clinical assessment skills, knowledge of the community, ability to work with multidisciplinary team and to work with clients/families in the home setting. Knowledge of aging and programs for seniors.

Responsibilities:

1. Must evaluate the potential client as a total person and identify the health limitations, which impede independent living.

2. Conducting in-depth assessment/reassessments covering medical, health, and rehabilitation concerns

3. Certifying level of care determinations

4. Performing physical assessments as necessary and interpreting clinical findings

5. Working closely with physicians and other health professionals

6. Developing care plans, implementing and monitoring services, consulting with the Social Work Care Manager, case recording and reporting.

7. Provides care management in accordance with departmental policy, program standards and patient needs by:

a. Completing assessment evaluation of a clients health and

psychosocial status utilizing all relevant information sources including the identification of issues that are specific to age, diagnosis and disabilities of the client population served, in accordance with departmental policy.

b. Participating as an active team member in care planning to

develop care plans specific to the clients needs.

c. Implementing interventions and services utilizing community resources in a timely fashion in accordance with departmental policy.

d. Maintaining and documenting regular contact with clients in accordance with departmental policy and providing ongoing support as needed.

e. Identifying, assessing and responding to crisis situations in a timely fashion, with appropriate interventions.

f. Observing all legal, departmental, and/or hospital regulations.

8. Provides community relations by:

a. Developing and maintaining positive community interactions building referral relationships in the community.

b. Recognizing and resolving problems in a timely and

appropriate fashion.

c. Participating in community outreach.

d. Seeking new resources.

9. Maintain effective interpersonal communication skills by:

a. Using open and accurate verbal and written communication.

b. Recognizing and resolving conflicts or unusual situations.

c. Maintaining positive interpersonal relationships and team building within department and ancillary staff and with community partners.

d. Completing written correspondence and documentation in a timely manner.

Physical Demands:

Lifting capability of 5-10 pounds (depending on what is taken in the field) laptop, paperwork etc.

Valid California drivers license and proof of current liability insurance required.

This position requires travel throughout Kern County for home assessments.
