SUMMARY:

The Medical Records Clerk is responsible for the integrity of patient health information contained in the patient chart and ensures that it is organized and placed in the chart within a timely and accurate fashion. This position is also accountable for providing excellent customer service to patients and is well versed in the understanding of confidentiality and the release of information requirements as mandated by State and Federal law. Staff members who occupy this position are expected to continually update his/her computer and service knowledge skills in order to function effectively with Electronic Health Records (EHR) and other Practice Management Software used at Camarena Health.

DUTIES and RESPONSIBILITES

1.0 Focus on Patient Records:

1.1 Support patient service delivery by actively engaging in following the expectations to organize the patient's information according to policies and procedures.

1.2 Support the team by maintaining records electronically. Should have a general understanding on how to complete a batch scan, single scan, index, and referral activity.

1.3 Supports the accurate collection of patient information by ensuring that forms, when scanned into the health-record are appropriately indexed and that scans are clear for ease of review.

1.4 Actively support the health center by regularly printing disease tracking follow-up letters for Front Support to follow-up/ send out.

2.0 Focus on Patient Flow:

2.1 Supports the service delivery area by ensuring that records are easily available to staff and providers.

2.2 Supports other teams by collecting and ensuring documentation for upcoming OB scheduled appointments which includes following up with consulting providers.

2.3 Supports CPSP Case Managers by routing patient follow up alert request and by adequately tracking labor and delivery records in order to ensure timely closure of cases.

2.4 Acts as a facilitator to implement simplified methods of trafficking documents between several sites-- mail, release of information requests, etc.

2.5 Supports Camarena Health release of records policies by controlling internal and external medical records requests. Responsible to assure timely completion of forms, and release of medical & dental records requests.

3.0 Focus on Medical Records Teamwork:

3.1 All Medical Records Clerks act as a back-up to each other by learning and assisting with all medical record functions.

3.2 Learns other functions such as Patient Referral Specialist (PRS) and tracking, as eligible through aptitude, skill, present performance levels, and initiative.

4.0 Focus on Corporate Expectations/Standards:

4.1 Attends and actively participates in all meetings (e.g., team meetings, department meetings, program meetings, employee staff meetings) and other activities as required or assigned.

4.2 Attends workshops/seminars as necessary to increase skills and knowledge to provide effective support.

4.3 Works flexible or extended hours where necessary.

4.4 Demonstrates awareness of, and compliance with, organizational mission and objective of Camarena Health to provide health care access and support services for all members of the community.

4.5 Other work-related duties as assigned by supervisor; duties and responsibilities may be added, deleted, or changed at any time at the discretion of management, formally or informally either verbally or in writing.

4.6 Maintains confidentiality and respect for information regarding patients and other team members; abides by Camarena Health Rules of Confidentiality

MINIMUM REQUIREMENTS:

Education:

• High School Diploma or GED.

• Some College Preferred

• Completion of Medical Terminology Coursework Preferred

• Strong understanding of ICD-9 coding guidelines

• Strong understanding of CPT coding guidelines

Prior Experience:

• Experience equivalent to two years in medical records or general/medical office procedures

Skills:

• Bilingual (English-Spanish) preferred.

• Excellent oral and written skills.

• General medical terminology and standard abbreviations used in medical notations.

• Telephone courtesy; customer-service oriented.

• Modern office practices and procedures including email.

• Intermediate to expert user computer skills.

• Highly organized.

Physical Requirements:

• Must be able to move up to 20 pounds and push up to 50 pounds (on wheels)

• Must be able to hear staff on the phone and those who are served in-person, and speak clearly in order to communicate information to clients and staff

• Must be able to read memos, a computer screen, personnel forms and clinical and administrative documents

• Must have high manual dexterity

• Must be able to reach above the shoulder level to work, bend, squat and sit, stand, stoop, crouch, reach, kneel, twist/turn
