Youth Support Coordinator I Position Description

Job Title: Youth Support Coordinator I

Hours: 30-40 hours per week

$19.00 - $21.00/Hr. (with 5% pay differential added to hourly base rate for staff who are Bi-lingual and Bi-cultural, available after 90 day introductory period)

Benefits: Medical, Vision, Dental (up to $300) within 90 days of hire date

Supervisor: YES Program Manager

Introductory Period: 90 days

Note: This is an at-will position, contingent upon funding availability

Summary:

Under direct supervision of the YES Program Manager, with support from Youth Support Coordinator II positions, the Youth Support Coordinator I will offer direct service and support to youth and transition age youth (ages 12 to 26) who are currently involved in, emancipating from, or returning to Systems of Care services (Mental Health, Child Protective Services - known as CPS or CWS, and/or Probation) and residing in Placer County. Primary responsibilities will be: educating and supporting participants about different systems, resources, services and/or placements and how to navigate them; assisting and empowering participants in developing, implementing and advocating for their individual transition goals; supporting participants in their system and community meetings; utilizing participant feedback, as well as your direct personal experience as a recipient of system or related services, to influence community and system transformation.

Requirements:

Must be 18 years of age or older and no longer receiving services from Children’s System of Care.

Strong work ethic, organization skills, dependable, flexible and able to adapt to daily changes and challenges.

Must have direct personal experience as a youth recipient of services such as, mental health/counseling, probation, Wraparound and/or Child Protective Services/CPS.

Knowledge of and/or ability to learn Positive Youth Development, Motivational Interviewing, Strengths-Based Practice and Cultural Sensitivity.

Ability to work cooperatively and effectively with individuals and groups from different educational, economic, cultural and racial backgrounds.

Proof of a minimum of 100/300/100 Liability Insurance Coverage is required prior to driving personal vehicles on company business.

Must own and effectively maintain a personal vehicle and possess a valid State of California Driver’s License, with approved DMV Record, in order to fulfill the essential functions of this position.

Must pass an extensive criminal background check and drug screen.

Bi-lingual and Bi-cultural highly recommended, with pay differential available after 90 day introductory period.

Job Duties:

Provide education about different systems, services and/or placements and support in how to navigate them effectively.

Support youth in court and/or collaborative teams (e.g., Individualized Education Plan (IEP) meetings, Child and Family Team Meetings).

Inform youth of their rights within the system and how to advocate for their rights.

Research and connect youth to available community services/resources.

Meet with and/or transport participants to locations in the community (e.g., schools, job training agencies, coffee shops, etc).

Teach/empower youth to use self-advocacy skills and engage in the decisions that affect their lives (e.g., placement changes, transition out of care).

Work collaboratively with each youth’s individual support team to ensure youth voice and choice, unconditional positive regard for families, and identification of and connection to permanent lifelong connections with natural supports.

Assist in the design and facilitation of groups, presentations, events and/or trainings.

Effectively communicate and represent participant feedback and your direct personal experience at various public meetings and workgroups related to systems change (e.g. Campaign for Community Wellness, System Improvement Plan, etc.).

Act as a positive role model for youth and encourage positive and healthy choices.

Promote a positive agency image to youth, staff and other community members.

Ensure confidentiality is maintained at all times and in accordance with Federal, State and Agency standards.

Complete and submit accurate documentation as assigned in a timely manner and in accordance with policies and procedures.

Attend and participate in required trainings, staff and supervision meetings.

Actively collaborate and coordinate activities with County and community-based organizations, such as Sierra College, Golden Sierra Job Training Agency, etc.

Empower and support participants to participate in decision making meetings and

identify their needs regarding placement.

Reduce stigma around youth at risk of, in or emancipated out of system services by working in partnership with county and community partners.

Perform other duties as assigned by your Supervisor.
