We have a position in our MSSP Program, working at our Eureka location. Our Social Work Care Manager is responsible for evaluating program participants and identifying the functional limitations which impede independent living. The SWCM conducts in-depth assessments covering psycho-social, rehabilitation, and environmental concerns. The SWCM refers participants to special consultations to provide detailed evaluations and recommendations to improve the participant's functional level. The SWCM develops care plans, implements services, and conducts follow-up and monitoring of the plan. Our opening is for a Part-time Social Work Care Manager (32 hours per week), working day shifts, Monday-Friday.
