The Quality Improvement (QI) Coordinator will assist with quality improvement programs and collaborates with the leadership team and supervisory staff to aid with developing quality improvement measures. Responsible for assisting with implementation and monitoring the RRHC Quality Improvement Program (QIP), Partnership Healthplan of California Quality Improvement Program, and NCQA Patient-Centered Medical Home (PCMH) recognition. The QI Coordinator will assist the QI Analyst to investigate incident reports, patient complaints, and patient care issues.

ESSENTIAL FUNCTIONS

Assists QI Analyst in developing, implementing, and monitoring RRHC’s annual QI Plan.

Maintains and monitors annual QI outcomes and compliance calendar of required projects and reports.

Works with QI Analyst to assure necessary data collection and analysis are met for all quality improvement and compliance projects, including but not limited to PHC QIP, PCMH, FTCA, i2 tracks, incident reporting and UDS reporting.

Assists with the preparation of bi-monthly quality improvement committee meetings.

Update and Maintain detailed records on quality improvement scores for all measures and departments.

Collaborates with the leadership team to conduct ongoing evaluation of the validity and reliability of quality improvement measures.

Complete detailed monthly reports highlighting quality improvement outcomes for all departments and locations. Reports are to be posted in employee areas and e-mailed to RRHC’s leadership team and board.

Monitors quality improvement, utilizing evidence-based quality improvement measures.

Help identify trends, prioritize, and direct others in implementing recommended improvements.

Ensures compliance with state and federal regulations pertaining to patient safety and quality improvement.

Under the direction of the QI Analyst investigates incident reports, patient complaints, and patient care issues, providing direction for facilitation of improvements to patient safety based on identified risks.

Informs team of results of approaches taken towards meeting quality goals.

Other clerical support as assigned.
