RN NEEDED FOR 4 ICFID-H AND N FACILITIES IN THE INLAND EMPIRE.

Our facilities are Intermediate Care Facilities (not SNFs) for the Intellectually Disabled, with both Habilitative (ICF/ID-H) and Nursing (ICF/ID-N) homes
Our ratios:
Caregiver to client ratio does not exceed 1:3 and is sometimes as high as 1:1
Nursing caseload is 24-30
Our home sizes are small (6-bed)

Complex case management will keep you engaged and learning
High-acuity care is limited or prohibited by facility regulations
Your responsibilities will include, but not be limited to the following:
Provides direct RN assessment and care management of each resident minimum 1-1.5 hours per week
Quarterly nursing exam with MD
Reviews and implement feeding plans
Provides complex care management and vital assessments
Develops health care plan
Supervises LVN clinical responsibilities
Accompanies clients to medical appointments when warranted
Ensures doctors' orders are carried out
Case Management Responsibilities Include:
Completes required nursing reports includes monthly summary, quarterly reports HCP and other
Determines LVN assignment and schedule
Participates in IDT (Inter-disciplinary team) meetings
Works with Pharmacy consultant for Pharmacy audits
Audits MAR (Medication Administration Record) bi-monthly
Assists monthly in-service training
Oversees and support monthly cycle medications
Schedules doctors' appointments
Ensures specialist referrals and appointments are properly scheduled
Prepares client charts for doctor rounds
Receives and record doctors' orders
Assists physician on Dr Days
Qualifications:
Valid California RN License
Excellent oral and written skills
Strong critical thinking skills
Ability to provide complex case management for medically involved and fragile clients
Experience working with adults with disabilities and/or intellectual disabilities preferred
Benefits and Pay:
Hourly $35.00
Sign on Bonus $1500 ($750 upon hire and $750 after completing your first year)
