Five years post graduate social work experience

Strong assessment and counseling skills
Preferred Skills and QualificationsHospice and/or hospital experience
LCSW certification
Bilingual English/Spanish

This position utilizes therapeutic counseling and skilled interventions to meet the needs of patients and families dealing with terminal illness. Provides community resource information to patients and families to meet their individuals needs. Comprehensively assess bereavement needs of families and utilizes therapeutic counseling to meet psychsocial needs of the grieving family. Effectively coordinates care with interdisciplinary team. Completes timely, thorough, individualized patient documentation.The social service department provides end of life counseling and resource services to the hospice patients and their family members. All social workers are master prepared and specialize in the area of death and dying. The department consists of medical social workers who work closely within an interdisciplinary model to provide a holistic approach to end of life care. All clinicians are provided cell phones and lap tops that so that communication and documentation is timely in the patient's electronic medical record. Sharp HospiceCare is dedicated to providing comprehensive care and compassionate support to individuals facing life-limiting illnesses, along with their families and friends. Services range from relief of physical discomfort to emotional and spiritual support. Sharp HospiceCare also provides “pre-hospice” home based palliative care consultation for patients with advanced illness such as heart failure, chronic obstructive pulmonary disease and dementia.Sharp HospiceCare provides in-home services and also provides the region’s first freestanding hospice homes, LakeView Home in La Mesa, ParkView Home in Del Cerro, and BonitaView in Bonita, which offers end-of-life patients an alternative to nursing-home placement. The homes — staffed 24 hours a day by licensed nurses — offer a comfortable and supportive environment for up to four to six patients at one time in each home.
