DSP's/ Community Facilitators
The DSP's/CF's provides vocational and/or skills training to persons with developmental disabilities.
Duties include:
- Follow Clients' Person-Centered Planning
- Assist client in selecting realistic goals and objectives
- Assist clients in participation in chosen club activities
- Assess, evaluate, implement and chart client's progress on goals and objectives
- Pick-up and drop off clients according to written schedules using public transportation
- Adhere to external employment site-rules, regulations and scheduled work hours
- Record data and maintain field books
- Follow all policies and procedures as they relate to job performance
- Provide constant supervision to clients
- Train and monitor clients in site specific safety procedures including infection control
- Accept changes in assigned clients or program sites as directed
- All other duties as assigned by the Program Manager
- Mandated Abuse Reporter
CNA's/ Caregivers
The CNA/ Caregiver will provide 1:1 assistance to individuals with disabilities.
Duties will include:
- Assist with activities of daily living, such as bathing, dressing, feeding and toileting.
- Provide support to patients with special needs.
- Maintain a clean and safe environment for patient.
- Assist with the setup and maintenance of durable medical equipment (DME).
- Follow infection control protocols and assist with sterile processing as needed.
- Assist with outings
Experience REQUIRED to work with individuals with disabilities
Bilingual in Spanish and English
