Job SummaryThe Behavioral Health (BH) Assistant works as a member of the behavioral health team to extend the services of the behavioral health provider and assist in managing patient flow related to Primary Care Behavioral Health Contacts. They also assist the behavioral health provider and population health team in providing on-going care for patients at risk for poor outcomes. This may involve management of patient registries and initiation of on-going contact under the direction of the behavioral health/primary care provider with identified patients until improvements are made. The BH Assistant will also patients connect to external agencies that can help with identified social service needs. What You'll Do:Be available to primary care patients and health professionals on a same-day basis.
Provides triage and patient flow assistance to the Behavioral Health Provider (BHP).
Administers standard screening and outcome measures instruments.
Facilitates medical provider and patient requests for same-day and future consultations with the BHP.
Provides requested interventions related to chronic disease/behavioral health management pathways.
Manages and coordinates referrals to specialty behavioral health services including Mental Health and Substance Abuse services.

Qualifications:High school diploma required, BA/BS in Psychology or Social Work preferred.
State of California Certified Nurses Assistant (CNA), Medical Assistant, or Emergency Medical Technician (EMT) required.
American Heart Association BLS certification required.
Job Information:Full-time, day shifts.
