A cash allowance of approximately $600/Month will be added to the starting pay

Plus, an additional $1.15/Hour premium for bilingual assignments

The Human Services Department's Adult & Aging Division is currently recruiting for positions in the following programs. Spanish/English speaking candidates are encouraged to apply.

Information and Assistance (I&A)

The I&A program provides resources and follow up to older adults, people with disabilities, and caregivers, primarily through a public hotline (565-INFO). This service is available to all individuals in Sonoma County seeking access to aging and disability services. The ideal candidate will have:Significant experience working with older adults and people with disabilities in the community

Strong interview skills, including by telephone, in a fast paced environment

Aptitude for performing brief needs assessments while screening for programs and services that will assist the caller

Experience working with individuals in emotional crisis

A high degree of comfort providing counseling and resources to address immediate needs

Skill in providing community outreach, and presenting to community groupsHome and Community Based Alternatives (HCBA) Waiver

The HCBA Waiver program supports Medi-Cal beneficiaries' goals to return to (or remain in) the community setting of their choice. This position works with clients and family members, among others, to determine what services are needed for clients to stay at home as safely and independently as possible. They provide comprehensive case management, as well as home and community-based services/supports, to eligible Medi-Cal beneficiaries of all ages who would otherwise receive care in an institutional setting.

NOTE: The California Department of HealthCare Services requires employees assigned to this position to have obtained a Masters in Social Work (MSW). The ideal candidate will have:Extensive case management experience working with clients who have developmental and/or physical disabilities

Experience working in either a Skilled Nursing, Acute Care, or Rehabilitation facility doing discharge planning

The ability to work independently and to collaborate as part of a high functioning team

Excellent written and oral communication abilities
