Hospice Services of Lake County is seeking a contract Physical Therapist to evaluate and treat the physical disabilities of patients, with a focus on maximizing independence. The incumbent of this position functions as a member of the interdisciplinary team with expertise in physical therapy.
Graduate of a college/university program in Physical Therapy that is approved by the American Physical Therapy Association; and possess a current California Physical Therapy License.
One (1) year experience in physical therapy in an adult rehabilitation setting, within hospital, home health and/or skilled nursing center.
Demonstrates good verbal and written communication, and organization skills.
Possesses and maintains current CPR Certification.
Understanding of hospice philosophy and issues of physical mobility for terminally ill individuals.
Must be a licensed driver with an automobile that is reliable and insured in accordance with state or organization requirements.
