PROGRAM: Lake County Transitional Home (Lake House)

Lake County Transitional Home, a licensed residential facility, is a cooperative effort between Turning Point of Central California, Inc. and Redwood Coast Regional Center. Lake County Transitional Home is available to clients eligible for Regional Center services who are between the ages of 7 and 17, can serve up to four youths. It has individual rooms for youths with intellectual disabilities and a co-occurring mental health diagnosis, and is designed for long term stabilization. The program focuses on positive reinforcement, social integration into the community, and an emphasis on teaching age-appropriate life skills that the youth will need to maximize independence as they reach young adult hood. Transition planning is key and on-going in order to support the youth as they reach adulthood to be successful in the "adult" environment they transition into. This will be coordinated with the Service Coordinator and family to occur when the youth turns 18 years of age.

JOB SUMMARY

The Program Director reports to the Regional Director and is responsible for the coordination of direct client care and client activities of the facility while assisting in the supervision of all agency program staff as it pertains to direct client care.

ESSENTIAL JOB RESPONSIBILITIES

1. Development of program activities and program documentation procedures.

2. Assist in the direction of the resident's treatment as outlined in the resident's Person Centered Plan in compliance with the Program Design.

3. Transport clients as needed.

4. Personnel Management

5. Recruit and screen facility staff in accordance with Turning Point of Central California, Inc. policy and procedures.

6. Make recommendations in regard to employing and terminating staff.

7. Assist in the supervision of program staff regarding their skills, overall functioning and program management.

8. Produce monthly reports as required not limited to treatment summaries, discharge summaries, or alteration of residents' goals and outcomes.

9. Work with Social Worker to assure compliance with proper chart documentation.

10. Assist in the provision of direct services to clients-including working shifts to ensure adequate coverage if directed by each resident's Person Centered Plan.

11. Enter significant information related to client's treatment in resident's file.

12. Assist in the coordination of the Person Centered Plan and individualized service plan for each resident.

13. Oversee the admittance of all clients and that relevant admitting paperwork/information is complete.

14. Attend relevant meetings, specifically participation on the treatment team assigned to the facility.

15. Determine staff assignments to ensure the facility is staffed 7 days per week per contract.

16. Approve and monitor expenditures and revenues, including client's Purchase Authorization terms.

17. Monitor facility food and supply stocks to maintain compliance with CCL regulations.

18. Be present at the facility to resolve any major problems, including neighborhood complaints if any.

19. Will drive on Agency business and transport clients if needed.

20. Maintain regular on-site hours for successful oversight of program.

21. Will think and act quickly and efficiently in emergencies.

22. Comply with all Turning Point Safety policies and procedures, including but not limited to: workplace safety, reporting work related injuries, Infection Control and preventing potential safety risks for staff, clients and others.

23. Employees are expected to manage their cell phone use so that the Agency cell phone stipend covers all business usage. Any calls not covered by the stipend are to be made on a company landline phone.

24. Punctual and regular attendance is an essential responsibility of each employee at Turning Point. Employees are expected to report to work as scheduled, on time and prepared to start working. Employees also are expected to remain at work for their entire work schedule. Late arrival, early departure or other absences from scheduled hours are disruptive and must be avoided.

25. Other duties as assigned by the Program Director or Regional Director including covering for direct care staff in the event of staff shortage. ESSENTIAL JOB REQUIREMENTS

1. Bachelor's degree in Social Work or Counseling, or a related field and at least one (1) year direct experience providing social services to children OR completion of two (2) years at an accredited college or university and at least two (2) years administrative or supervisory experience in a program serving children, preferably in a group home setting.

2. Minimum of one (1) year experience with the developmentally disabled.

3. Ability to supervise staff and work effectively with developmentally disabled system clients.

4. Current certified training in CPR/First Aid procedures.

5. Completion of either Pro-Act training within the first three months of employ.

6. Meet the requirements for CCL Group Home Administrator Certification within the first year of employ.

7. Intermediate computer skills in Microsoft Word, Excel, Outlook and PowerPoint.

8. Possess a valid driver's license and have access to a dependable means of transportation that is properly insured and operated in accordance with all laws. Agency insurer authorization is required.

9. Ability to pass a criminal background check.

10. Ability to physically ascend and descend stairs on a frequent basis.

11. Ability to pass a pre-employment physical, drug screen, general physical, and TB test.

12. Must maintain good relationships with consumers, co-workers, government and civic representatives, as well as community members within whom the Agency is transacting business, and relate to them in a professional manner.
