Position Description

Title: Specialty Mental Health Care Manager I / II / III

Reports to: Therapist

Hours: Full-time

Salary Range: $16 - $20 per hour

General Duties and Responsibilities

Under the supervision of the Therapist, the Specialty Mental Health Care Manager I / II / III will provide service coordination through assessments, referrals, and follow-up to people with serious mental health issues accessing services at the Hospitality Center.

The Specialty Mental Health Care Manager I / II / III will work the caseload assigned. This position is required to work cooperatively with other services and programs, program partners, and numerous community agencies. The Specialty Mental Health Care Manager I / II / III) will develop, monitor, and support a case plan with each eligible client. Perform other duties as assigned.

Specific Duties and Responsibilities

Provide activities which increase the opportunity for clients to improve their physical, mental and emotional wellness and recovery.

Ensure that a safe, secure and welcoming environment is provided by maintaining safety protocols.

Ensure the provision of practical help and linkage to services.

Provide an intentional clinical experience for each client, derived from their Care Plan, utilizing Evidence Based practices.

Coordinate the provision of access to MCHC services through MCHC staff on-site and referral to other services, such as housing, education, welfare benefits & financial assistance.

Provide care management and supportive services to people with serious mental illness under Title 9.

Complete all required record keeping, including Medi-Cal billing and electronic and paper-based case records.

Deliver the productivity rate for the position (currently 63% billable time).

Utilize outcome management tools and other electronic care management tools.

Provide information and referrals for clients eligible for other services in the greater Fort Bragg area and Mendocino County.

Participate in weekly case conferencing meetings, HH/ HC staff meetings, and related training as scheduled.

Work to process and problem solve client grievances.

Maintain good working relationships with other service providers and other agencies.

Follow HH/ HC protocol for securing Release of Information from clients.

Maintain client confidentiality.

Maintain clients records and data, including inputting of data to the care management databases, including Medi-Cal related data.

Coordinate with other HH/ HC services.

Job Skills and Abilities

Care Manager III: ($18 - $20 per hour based on Experience.) An individual who has at least four (4) years of full-time/equivalent (FTE) experience in a mental health related field proving direct mental health services. Up to two (2) years of the four (4) years of experience can be substituted by years of education in a mental health related field. There is a minimum requirement of two (2) years of actual work experience.

Care Manager II: ($17 per hour) An individual who has at least two (2) years but less than four years of FTE experience in a mental health related field providing direct mental health services.

Care Manager I: ($16 per hour) An individual who has less than two (2) years of FTE experience in mental health related field providing direct mental health services.

Experience working with those with mental health issues.

Understanding of basic substance abuse and mental health diagnoses and treatments.

Ability to exercise good judgment, follow oral and written directions.

Good clerical skills and attention to detail

Ability to communicate effectively both orally and in writing.

Demonstrated ability to establish cooperative working relationships with clients and staff.

Understanding of and ability to maintain client confidentiality.

Excellent listening skills

Good computer skills including ability to use Office software, databases, and Internet.

Ability to maintain attitude of fairness and objectivity.

Knowledge of greater Fort Bragg and Mendocino County social and human service programs.

Valid California Drivers License + Clean DMV Record
