Job Summary: The Licensed Vocational
Nurse under the supervision of the Registered Nurse is responsible for the
implementation of the Plan of Care by administering specific nursing care to
patients in the home setting.Essential Functions:
Assists the RN withimplementation of the Plan of Care by performing and documenting nursinginterventions including patient response, patient education and safetyinterventions.
Completes a basic assessmentwithin scope of practice, and reports objective findings to the RN CaseManager or physician.
Reports effectively to the RNand documents changes in patients’ needs/problems. Communicates withpatient, families and physicians.
Reports any signs of treatmentcomplications, adverse medication reactions or changes in patientcondition to the RN, Clinical Manager or Physician.
Administers medications andtreatments according to Agency policy, accepted standards of care and homehealth care guidelines, following Physician orders.
Additional Requirements:
Participate in weekend, on-calland holiday rotation
Minimum Position Qualifications:
Completes necessary educationto attain LVN Licensure in the state of California
California LVN license
California Driver’s License andproof of Auto Liability Insurance
Basic Life SupportCertification
1 year of Licensed VocationalNurse experience
Preferred Position Qualifications:
1 year Home Health LVNexperience
Phlebotomy and/or IVCertification
