Summary:

The Parents As Teachers (PAT) Home Visitor is an integral part of the Integrated Services Collaborative (ISC) Integration Team and as such, will manage required activities for integration and navigation including: outreach, welcome, screening, participation in multi-disciplinary review team meetings, as needed, reflective supervision and documentation/data entry. The PAT Home Visitor will work closely with families with children from Prenatal to ages 0-3, with other members of the Integration Team, Collaborative and Community Partners, including CalWORKs staff.

ESSENTIALS DUTIES AND RESPONSIBILITIES:

Include the following as other duties and responsibilities that may be assigned

Outreach:

• Together with PAT ISC Care Coordinator, develop and distribute promotional materials to engage CalWORKs families of children ages 0-3 to participate in home visiting program.

• Provide orientation information sessions to CWES staff to familiarize them with Home Visiting services.

• On-going individualized outreach shall include phone call/text reminders and check-ins to encourage participation and a sense of connection with the program, and to maximize participation for those parents who may feel most vulnerable or isolated.

Welcome:

• Accept appropriate CalWORKs customer referrals from CWES for participation in this project via a PLAN 103 CIV form.

• Implement F5MC Parent consents and an “enrollment”.

• Use relationship-based strategies sensitive to family language, culture, and context that “meet the family where they are” to develop an understanding of family values, strengths, current services and goals.

• Connect with appropriate information, resources and assistance, with the Collaborative or through Community Partners.

Screening:

• Conduct child screening using ASQ3, ASQ-SE2 for participating parents/children.

• Administer assessment tools that adhere to the Parents As Teachers National Center quality standards.

• Use culturally attuned interactions and approaches for establishing a relationship of trust to gather information on the family context and set the stage for follow-up.

Home Visitation:

• Provide at least two (2) home visits per month for each participating CalWORKs family.

• Work with PAT Care Coordinator and CWES staff, as needed to ensure that there is support for parental access to home visiting services and to support focused parent/child attention for parents and their children under the age of three (3) years.

• Inform DSS case manager of recommendations for parent/child referrals, as needed.

• Provide case management and participate in selected case management meetings at designated DSS sites as needed.

• Follow through on referrals to support family access of the recommended service. Share relevant information with service providers and monitor confidentiality.

• Participate in F5MC data entry into the appropriate database to ensure accurate and timely monthly reporting.

• Participate in Reflective Supervision through small group or individualized meetings.

General:

• Engage in evaluation, monitoring and continuous quality improvement activities aligned with F5MC’s Strategic Plan Framework and PAT National Center.

• Engage in professional development, including attending at least 3 of the 6 Saturday sessions of the Infant Family Early Childhood Mental Health (IFECMH) training series, provided by F5MC.

• Coordinate data entry and management with ISC Data Coordinator.

• Work with ISC Data Coordinator to maintain time logs and document other administrative activities as appropriate for participation in the federal reimbursable programs Medi-Cal Administrative Activities (MAA).

• Maintain client files, including intake, family data and evaluation forms to ensure appropriate information is collected and appropriate HIPAA regulations are adhered to.

• Other duties and responsibilities as assigned.
