Nightingale Senior Care is seeking a care coordinator to manage the following roles:
New Client Intake.
Creating Care Plans for clients and keeping them current.
Ensure customer satisfaction for the clients (and the family member(s) responsible for their decision making)
Ensure customer satisfaction for the caregivers working with the company.
Working with discharge managers and social workers from skilled nursing facilities and hospitals on discharged patients needing in home care services.
Handling inbound phone calls from clients and caregivers.
Placing calls to clients and caregivers using company phones.
Driving to visit clients in their homes and at facilities they may temporarily be in.
Using a computer to enter data, create care plans and to communicate electronically (Email and SMS) with clients and customers.
This is an In Office Full Time Position with driving required while on duty.
Computer expertise NOT required, however, basic typing skills are required.
Occasionally assisting other office staff members with tasks such as scheduling, credentials management and recruiting.
HCA registration number preferred. Live Scan clearance with the California Department of Social Services is required. Hence, a live scan background check may be needed if you are not already in good standing with the California Department of Social Services.
