Responsible for coordination of care to meet identified Occupational Therapy needs of the patient/family; participates in the management of patient care to include identification of needs of Occupational Therapy intervention, discharge planning, resource identification, linking and scheduling patients with required services, monitoring care delivery, advocating for the patient, and evaluating outcomes, facilitating patient care across the health care continuum, and assists other team members with meeting patient needs. Is preceptor for related staff and students. Requires California State Occupational Therapist License. This position may require flexibility of hours. Education: Graduate of an educational program for occupational therapists approved by the California Board of Occupational Therapy and accredited by the American Occupational Therapy Association€™s Accreditation Council for Occupational Therapy Education (ACOTE). Experience/Training: Determined by patient care area. Minimum one year related practice experience preferred. Licenses/Certifications: California State Occupational Therapist License. Effective 10/9/2017, National Occupational Therapy Board Certification required. Current American Heart Association BLS provider required. Additional State credentials required for OTR/L working with modalities; specialized hand diagnoses patients; pediatrics; dysphagia required. Skills: Successful completion of orientation. Occupational Therapist working with pediatric patients: 1.Occupational Therapy provides pediatric assessment, treatment, and family/staff consultation and education services. Patients referred for Occupational Therapy by a physician will receive an evaluation that is appropriate to age, medical diagnosis, and family concerns. A treatment plan is developed from the evaluation findings. Age specific occupational therapy intervention is provided as defined by the individualized treatment plan. 2.Occupational Therapy is part of the multidisciplinary pediatric team. Team members will actively involve the family and/or primary care giver when establishing goals from initial assessment, through the course of treatment and in discharge planning. An occupational therapy representative will participate in team meetings regarding pediatric patients who are receiving occupational therapy intervention. 3.In collaboration with the pediatric department clinical nurse specialist and/or nurse educator, the occupational therapist will provide education and consultation for pediatric staff development regarding OT related topics pertinent to overall patient care. 4.Occupational Therapy collaborates with other providers to facilitate the continuum of care. Example: The pediatric OT will coordinate with a CCS case manager and/or a representative of the CCS Medical Therapy Program for referral to a CCS-paneled provider or CCS MTU for the patient with a CCS-eligible condition who may continue to require occupational therapy services after hospital discharge. The pediatric occupational therapist will communicate with other health, education and social service providers such as the Local Education Agency, Regional Center, or pediatric inpatient/outpatient programs to assure continuity of intervention. 5.Occupational Therapy is performed by occupational therapists (OT) and occupational therapist assistants (COTA) that are licensed by the state of California and certified by the National Board for Certification in Occupational Therapy. There is at least one occupational therapist that is CCS-paneled on the hospital staff.
