Job Description

Saturday and Sunday

Evaluates patients referred to the Organization; identifies insurance coverage, and coordinates referrals for home care services.

Essential Job Functions/Responsibilities

  1. Evaluates hospitalized patients referred for hospice.
  2. Identifies and verifies insurance coverage of hospice services.
  3. Obtains and documents prior authorization for hospice services from insurance providers.
  4. Collaborates with the hospital's discharge planning personnel, utilization review department and insurance Case Managers to facilitate safe, early discharge.
  5. Consults with physicians, nurses, social workers, discharge planners and other disciplines to establish a coordinated home plan of care.
  6. Interviews the patient, family, and caregiver and discusses the home situation, current needs, and any psychosocial factors that are relevant to the plan of care.
  7. Completes referral information that includes intake data, essential background information, hospital course, and the plan of care.
  8. Maintains liaison relationship with hospital and insurance personnel, providing information and education on Organization services, coverage issues and related areas.
  9. Participates in patient care conferences and in-services.
  10. Collects and maintains statistical data on all referrals and submits them regularly as required.
  11. Attends meetings and participates on committees as required.
  12. Participation in Quality Assurance/Performance Improvement Program and Hospice Agency sponsored training.
  13. Performs other duties as assigned.

Qualifications

  1. High School Diploma required.
  2. Minimum of one year of experience in home care and/or hospice agency.
  3. Demonstrates an ability to work with other health care clinicians, development of home plan of care, and knowledge of third party reimbursement.
  4. Self-directed and able to work with minimal supervision.
  5. Demonstrate excellent observation, observation, and written communication skills.
  6. Ability to multi-task and work within a team environment.
