Day to Day Responsibilities of this Position and Description of Project:
Case management, care coordination, and development of individualized care plan for member related to their health risk assessment. Nurse expected to complete care plans and present members at interdisciplinary care team rounds to ensure that the members plan of care meets their health care needs.
