SummaryThe RN Care Coordinator for Hematology/Oncology is a professional registered nurse (RN) who is responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. He/she is accountable for adhering to established policies, standards of care and standards of practice. This RN demonstrates leadership when providing and improving patient care. The incumbent reports directly to the Nurse Manager of the Medical Specialty Clinics.ResponsibilitiesThe RN Care Coordinator for Hematology/Oncology assumes the following roles and responsibilities: 1. Participates in performance measure improvement. 2. Provides direct patient care. 3. Triages and applies a collaborative approach in identifying, analyzing, and resolving patient care problems. 4. Coordinates care of patients consulted to the Hematology/Oncology clinics. 5. Coordinates care with the Infusion Clinic to provide optimal care for hematology/oncology service patients. 6. Serves as the key resource personnel for the Hematology/Oncology Veteran population, to assist them in navigating through the complex healthcare system. 7. Review, triage new consults. 8. Check voice mail messages every 2-3 hours and prior to and after the business day. 9. Communicates between healthcare providers of the Hematology/Oncology service and/or non-VA/other VHA facilities to coordinate care. 10. Collaborates with assigned clinic nurses and other employees to coordinate office visits, treatment, procedures with Hematology/Oncology patients. 11. Collaborations with Infusion Center personnel to coordinate infusions, radiation, and clinic visits. 12. Coordinates evaluation of patients undergoing bone marrow transplants. 13. Coordinates hospital admissions, as necessary. 14. Serves as a resource to the medical and nursing staff in coordinating care for hematology/oncology patients. 15. Meets and collaborates with an interprofessional team for tumor board meetings to discuss and determine plan of care. 16. Attends inpatient rounding with the hematology/oncology team. 17. Reviews lists of patients receiving chemotherapy with the oncology pharmacist. 18. Performs other duties as assigned by the Nurse Manager. Work Schedule: Monday - Friday, 8-hour shift, 00730 - 1600. Overtime, Weekends, and Holidays may be required to meet patient care needs. Specific working hours will be discussed during the interview. Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Preferred Experience: At least three (3) years' experience caring for patients with hematology-oncology conditions.- Minimum one-year experience in care coordination of procedures, treatments, and transition from inpatient to ambulatory services.- At least three (3) years' experience in critical care nursing (i.e., Cath Lab, ICU, Emergency Department).- Certification in Oncology, Ambulatory Nursing, Emergency Nursing, Critical Care Nursing.- Experience in leading a team of residents, fellows, physicians.- At least two years charge RN experience. Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience [If MSN obtained in a Bridge Program, no BSN is required]; OR a Doctoral degree and approximately 2-3 year's of nursing practice/experience. Physical Requirements: Heavy Lifting (45 lbs and over); Straight Pulling, Pushing, Walking, Standing, Repeated bending (8 - 12 hours); Reaching above shoulder; Use of fingers; Both hands and legs required; Ability for rapid mental and muscular coordination simultaneously; Near vision correctable at 13" to 16" to Jaeger 1 to 4; Far vision correctable in one eye to 20/20 and 20/40 in the other; Both eyes required; Depth perception; Ability to distinguish basic colors and shades of colors; Hearing (aid permitted); Emotional/Mental stability; Keyboarding and reading computer screen (8-12 hours intermittently over scheduled shift).EducationIMPORTANT: A transcript with graduation or conferral or completion date must be submitted with your application.
