A local Elder Care management agency is looking for an experienced care manager.
Duties:
Provide care management services to older adults.
On-going support of older adults and their families.
Provide information and referrals to other program services.
Provide educational and housing assistance to eligible clients.
Provide crisis intervention to clients as needed.
Provide advocacy and referrals based on clients' needs.
Develop and maintain outreach and linkages in the community.
Collaborate with community based organizations.
Transport clients to and from appointments as needed.
Qualifications:
Masters degree in social work, psychology or a related field is preferred.
Experience working with older adults preferred.
Basic computer knowledge and skills.
Must be able to work independently.
Must have reliable transportation.
Valid CA Driver's License.
Good driving record.
Negative TB test results.
Current CPR and First Aid certification required.
Must carry professional liability insurance.
Must be an independent worker, flexible hours.
Job Types: Part-time, Contract
