Hospice Care of the Coast, Inc is seeking certified Hospice Aide to join our team.

Essential Functions:

- Provide emotional support and compassionate care to the patient and family unit.

- Under the direction and ongoing supervision of the registered nurse, provide essential personal care assistant to the patient.

- Report any changes in the patient's condition or family situation to the RN in charge or the Hospice office.

- Practice Standard Precautions according to OSHA regulation and policy.

- Attend at least 12 hours of inservice education programs per year.

- Maintain current certification as required by the State.

Qualifications:

- High school Diploma.

- Must meet Federal Home Health Aide training requirements.

- Demonstrate experience as a Hospice Aide.

- Minimum 1 year experience as a Home Health Aide or Hospice Aide.

RN / Case Manager

The Registered Nurse (RN)/Case Manager is the member of the interdisciplinary team who is the pivotal person in identifying the physical, psychological, social and spiritual needs of the patient and family. The Registered Nurse (RN) initiates appropriate intervention and support for the patient and family upon admission and provides a continuously appropriate, comprehensive and responsive plan of care.

The Registered Nurse (RN) is responsible for:

-Complete initial, comprehensive and ongoing assessments of patient and family to determine hospice needs.

-Regularly assess and reassess the nursing needs of the Hospice patients and family.

-Develop an individualized plan of care, based on identified needs.

-Ensure and document appropriate use of the medications, durable medical equipment and supplies.

-Educate patients, families ad other caregivers.

-Participate in Interdisciplinary Team Meeting as indicated.

-Perform specific nursing procedures as needed (e.g., treatments, management of symptoms) following the doctor's order.

-Participate in QI assessment routinely.

Qualifications:

-Minimum 1 year experience as a professional nurse within the last 3 years.

-Currently licensed as Registered Nurse (RN) in State of California.

-Ability to work independently, make accurate, and at times, quick judgments.

-Ability to supervise other appropriately.

-Acceptance of and adaptability to different social, racial, cultural and religious modes.

LVN

The Licensed Vocational Nurse (LVN) is responsible for providing direct patient care under the supervision of a registered nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes.

Essential Functions:

-Provide direct patient care as defined in the State Nurse Act for LVNs.

-Implement current nursing practice following a patient assessment and an approved plan of care initiated by the registered nurse.

-Provide accurate and timely documentation of patient services to reflect the plan of care.

-Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self plan of care.

-Use equipment and supplies effectively and efficiently.

-Evaluate patient's response to treatments/medications.

-Participate in the on-call rotation and weekends.

Qualifications:

-Minimum 1 year of experience as an Licensed Vocational Nurse (LVN) within the last three years.

-Currently holding a valid non restricted California Licensed Vocational Nurse (LVN) license.

-Good understanding of philosophy of Hospice concepts. Experience with the terminally ill patient (preferred).

Medical Intake Coordinator

Essential Functions:

Files information such as nurse's notes, patient assessments, process notes, laboratory reports, x-ray results, correspondence, etc. into patient chart.

Collect, assemble, check, and file patients charts as required.

Assist in developing procedure to ensure patient records are properly completed, assembled, coded, signed, indexed, etc. before filing.

Establish a procedure to ensure patient charts/records do not leave the medical records room except as authorized in our policies and procedures.

Maintain the confidentiality of all patient care information including protected health information. Report known or suspected incidents of unauthorized disclosure of such information.

File active and inactive records in accordance with established policies.

Batch patient information into the computer and retrieve patient demographic information as appropriate or as instructed.

Qualifications:

Minimum 1-year experience as a medical intake specialist within the last 3 years.

Currently licensed as LVN in State of California.

Ability to work independently, make accurate, and at times, quick judgments.
