OVERVIEW: Interfaith Community Services was founded in 1979 by a handful of diverse faith communities to address the needs of low-income, homeless and under-served people in North San Diego County. Interfaith offers programs through North San Diego County, from the coastal City of Oceanside, to the inland communities south and east of our Escondido headquarters. With more than 300 member faith centers and 130 dedicated staff, Interfaith serviced more than 16,000 unique individuals last year alone.
WHAT YOU WILL DO: This innovative position works in collaboration with the City of Escondido and the Community Oriented Policing and Problem Solving (COPPS) unit to address the issue of homelessness in Escondido.
Provides direct services to homeless clients and recently housed individuals and families, which include: intake, assessment, and case management as well as client advocacy with the goal of supporting self-sufficiency
Works in collaboration with the Citys COPPS unit to further enhance the work of the existing COPPS unit and provides comprehensive services to residents within the city experiencing homelessness
Develops trusting relationships with individuals and families experiencing homelessness
Completes a comprehensive biopsychosocial assessment and develops a detailed treatment plan for each individual
Utilizes motivational interviewing techniques to understand, assess, and shift the individuals stage of change
Helps stabilize individuals by linking them to appropriate resources such as housing, medical, mental health, addiction and recovery services, food, clothing, transportation, employment, Social Security Disability, mainstream benefits and other financial resources based on the clients individualized plan
Advocates to remove system barriers and to help individuals achieve their stated goals
Assists clients experiencing homelessness to identify housing options based on the individualized plan, and works to obtain and maintain permanent housing with the client for a period of up to nine months after housing placement based; develops a long-term support plan to ensure previously homeless residents maintain housing and continue to achieve other personalized goals
Works collaboratively with the COPPS unit, other service providers, and community partners to identify homeless individuals in need of case management services
Completes accurate and thorough documentation in a timely manner for each program participant, and provide required reports
Participates in case management meetings, staff meetings, trainings, conferences, program evaluation, and program development
Performs additional duties as assigned
EXPERIENCE / EDUCATION
Must be currently licensed by the State of California as a Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT) or Licensed Professional Clinical Counselor (LPCC)
3+ years of case management related experience required
Experience working with severely mentally ill, medically fragile, and homeless populations required
Bachelors Degree in Social Work, Psychology or related behavioral health field preferred
Bilingual (English/Spanish) preferred
ADDITIONAL REQUIREMENTS
Ability to establish and maintain professional boundaries with clients
Demonstrates a strong working knowledge of resources in North County San Diego
Must have reliable transportation and possess and maintain a valid California drivers license, including proof of personal vehicle insurance coverage and insurability under the agencys insurance carriers standards
Must successfully pass a criminal background and exclusion/disbarment check
Must successfully pass a pre-employment physical, tuberculosis test, and drug screen
Must successfully pass an Escondido Police Department background check
