PATH Santa Barbara is an integrated service center dedicated to assisting homeless individuals improve their health, find employment and transition to permanent housing. PATH Santa Barbara is open 24 hours a day, 365 days a year. PATH Santa Barbara has 100 interim housing beds year-round and from the months of December to March, the organization provides an inclement-weather winter shelter program with an additional 100 beds.About the Position:

In partnership with Cottage Health, PATH Santa Barbara manages an onsite Respite Care Program which provides assistance to individuals recovering from debilitating health issues, many of whom have recently been discharged from the hospital and have come to PATH Santa Barbara to fully recover.The Respite Care Monitor supports overall shelter operations management and provides assistance to a specific resident population: vulnerable individuals recovering from health issues. Supports case management staff on individual service plans, helps operate and monitor all elements of shelter, maintains orderly and clean resident bed areas in conjunction with residential staff. Serves the residents in their assigned living area as a member of the direct care team. Represents the member in facility meetings, committees, and serves as the first line for mitigating concerns with meeting the resident's needs.In addition to the hands-on experience with those with special care needs and their monitoring, other duties and responsibilities are engaged within this position. These additional duties include ensuring the well-being of all residents, administrative duties, support of the community kitchen, assisting other staff with projects, and overall facility cleanliness.Job Description:The Respite Care Monitor performs the following duties that are inclusive of, but not limited to:

Provides thorough patient care including hygiene, nutrition, ambulation or re-positioning, elimination and oxygen. Includes assistance with bathing.

Able to perform skills in accordance with Nursing Assistant checklist.

Demonstrates basic understanding of the concepts/disease processes most commonly seen in an emergency room.

Understands and utilizes universal precautions, and isolation techniques.

Handles and clean episodes of incontinence.

May attend and contribute to patient care conferences.

Provides individualized care as is determined by a registered nurse and hospital discharge paperwork

Monitors patients and report status and/or changes

Monitors and assist patients with food and liquid input/output

Assists with tracking and recording when residents self-administer their medications

Additional Information:

The Respite Care Monitor is an integral team member. In addition to his or her care duties, the Respite Care Monitor is also responsible for completing many smaller tasks. Such tasks might include entering data into a spreadsheet for grant reporting purposes, helping to ensure residents are completing assigned chores, assisting in administrative procedures for each shift, and helping to maintain the cleanliness of the facility. This work that he or she performs is nevertheless crucial to the overall success of PATH Santa Barbara. For this reason, the successful Respite Care Monitor will learn how to work well in a team, have great attention to detail, and a strong work ethic.Qualifications:

Certified Nursing Assistant (CNA) preferred or comparable certification including an Emergency Medical Technician with bedside experience

Ability to work as a team member

Ability to work in a fast-paced environment with a highly diversified population

Excellent verbal and written communication skills

Ability to maintain professional demeanor and attitude in a variety of stressful situations

Experience working with the homeless or similar population

Employment eligibility verification.

Reliable transportation.

Updated tuberculosis test.

Successful completion of background screening.·

Able to work alternative work schedules, evening and weekends are needed for operations of the Interim Housing Program.

Work Environment The employee may be in contact with individuals and families in crisis who may be ill, using alcohol and drugs, and who may not be attentive to basic personal hygiene, health and safety practices. The employee may experience a number of unpleasant sensory demands associated with the client’s use of alcohol and drugs, and lack of personal hygiene. The employee must be ready to respond quickly and effectively to many types of situations, including crisis situations and potentially hostile situations. The noise level in the work environment is usually moderate in an office setting. Sometimes work may become stressful when working under pressure.Physical Demands The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
