As a Patient Care Liaison, you'll use your clinical experience to process patient referrals to successful admission, including working with families, physicians, and clinical staff to facilitate smooth transition from hospital/nursing facility to home, building strong relationships with referral sources, and providing education on our services. This job is ideal for someone who is a high-energy people person, is a critical thinker and detail-oriented, and who has excellent communication skills. You must follow regulations and protocols correctly, work independently and collaboratively, and deliver great customer service with a caring and positive attitude.
RN/LVN required; experience in home health or hospice, as well as experience as a liaison or marketing position, a plus.
