POSITION SUMMARY

The Supportive Community Services Program located in Santa Maria, will deliver treatment, rehabilitative and supportive services to 85 clients with Severe and Persistent Mental Illness, “in vivo” in regular community settings (e.g., home, apartment, job site). For all Supportive Community Service Team clients, functioning in major life domains presents significant personal difficulties. These domains include affordable, safe housing; meaningful daily pursuits, including employment; and satisfying interpersonal relationships. Addressing the rehabilitation needs of clients in these key domains will be the Supported Housing Team’s essential purpose. The Supportive Community Service Team will include an on-site Behavioral Wellness medication provider, two nurses, Team Leader, Lead Clinician, Office Coordinator and four Rehabilitation Specialist staff and 1 Clinical Rehabilitation staff. The foundation of the Supportive Community Services Team shall be integrated treatment, rehabilitation and support services.

The Clinician must be English/Spanish bilingual and will be responsible for providing a range of therapeutic services including individual/group therapy, family education/collaboration, clinical assessment and treatment planning. The Clinician will meet with the Team Leader regularly to review a designated list of SCS clients to evaluate and perform clinical assessments. The Clinician will meet with clients and work with them to develop an individualized treatment plan and will be providing individual and group therapy. They will also be required to write supporting progress notes and other related documentation per Medi-Cal standards and BWell policies and procedures. All documents will be entered into the BWell electronic health record – Clinicians Gateway. The Clinician will report to the Team Leader but will work in conjunction with the other team members when developing assessments and treatment plans, scheduling, etc.

HOURS

Part-time, 20 hours per week. Schedule may include some evenings and occasional weekends.

MINIMUM REQUIREMENTS

Master’s Degree and California BBS current licensure. Must be English/Spanish bilingual. Well defined organizational and time management skills. Strong interpersonal skills, collaborative work style and oral/written communication skills. Ability to work independently and flexibly. Ability to handle intense situations, display good judgment and decision making skills, and remain calm and clearheaded under pressure. Able to work effectively with a variety of people and personality types regardless of age, ethnicity, race, religion, sexual orientation, gender identity/expression or socio-economic level. Must use reasonable precautions in the performance of one’s duties and adhere to all applicable safety rules and practices while acting in such a manner as to assure at all times, maximum safety to one’s self, fellow employees, clients, and other persons contacted through TMHA business. Conditions of employment include: Must be at least 18 years of age; possess a valid CA Driver’s License, a minimum of 2 years driving experience, reliable transportation, proof of auto insurance, and DMV driving record that meets TMHA’s current vehicle insurance requirements. May be required to utilize personal vehicle and auto insurance. Ability to possess and maintain good physical and mental health. Attendance is an essential function of this job that requires face-to-face interaction with clients and/or other employees. Comply with a T.B. skin test or chest x-ray indicating a negative tuberculosis result and receipt of a criminal record clearance through fingerprinting. Must be able to successfully complete First Aid and CPR training.
