Case Manager Specialist
We are interested in hiring a Case Manager Specialist to join our team and work in a new and exciting project (Por Vida Project). You should have previous experience as a Case Manager Specialist or similar classification. You will need to apply knowledge, skills and abilities in an educational setting with high school students and their parents. You must also exhibit relevant experience in working with students and parents to assess and develop program plans. Since parents will be a significant part of the Por Vida Project, the case manager specialist will provide parents with the appropriate program curriculum (Parents on a Mission.) The case manager will attend all Por Vida Project monthly panel meetings. All prevention/intervention services must be culturally and linguistically relevant. Case Manager Specialist must be trained in National Compadres Network Currilculums.
Case Manager Responsibilities include:
Assessment and management of student caseload on a regular basis.
Establishing and maintaining effective relationships with key community stakeholders.
Proactively development and maintain open communication with providers.
Working in accordance with relevant education legislation, behavioral health care best practices, and internal policy and procedures.
Accurately update and maintain cases and gather data for review.
Maintain accurate, up-to-date case information.
Help to manage student educational needs and other adjunct services in areas such as mental health, substance abuse, and other community resources.
Work with students to create a mutual service plan that they can accomplish.
Work with service providers on discharge planning when appropriate
Case Manager Requirements:
Bachelor’s degree in social work or related field is preferred but will consider job experience on a year per year basis.
Experience as a case manager specialist or similar related job description.
Experience working with students in educational setting to develop a service plan.
Sound knowledge of legislation relevant to Por Vida Project cases.
Valid driver license.
Background in health care, social work, mental health, substance abuse recovery.
Must demonstrate proficient computer skills.
Excellent communication skills, both written and verbal.
Must pass a Department of Justice Background Check.
Salary and Hours:
1. Position is a full-time (40 hours per week)
2. Rate of pay is dependent on experience
