ABOUT THE POSITION:

As Social Work Care Manager for Hospice of Santa Barbara, your primary responsibility will be to facilitate the planning, implementation and monitoring of a plan of care for an assigned group of patients and/or families. This role is a full-time position (40 hours per week) and includes health insurance, retirement, vacation and sick time and holiday benefits. As key member of the Patient Care Services (PCS) team, you will utilize your specialized training to provide compassionate care and support to those facing circumstances related to illness and/or end of life. The services you provide will be instrumental both in our Santa Barbara office and in Carpinteria, as our agency expands services into new territory. Through your meaningful contribution, the Patient Care Services (PCS) program will continue to blossom, answering the needs of our community. Specifically, your essential duties and responsibilities will be as follows:

Perform an initial, comprehensive psycho-social, spiritual, physical assessment for patients and/or families. Reassess periodically. Provide guidance, as needed, on entitlements/benefits or other financial or community resources.

Identify and integrate the psycho-social issues into goal-directed plans of care, working in collaboration with the PCS team.

Advocate for needed services and entitlements, obtain and connect to additional resources, intervene in crises, provide emotional support.

Perform short-term and long intervention and individual and/or family counseling as appropriate.

Collaborate with Director of Volunteer Services, Patient Care Volunteers and other team members to support the PCS program, including presenting in volunteer training modules.

Raise the community's awareness about the emotional, social and spiritual healing opportunities death and grief through community presentations; effectively communicate HSB's unique services to medical and social services persons and agencies for the purposes of creating a pipeline of referrals for HSB's PCS program.

Prepare and maintain electronic clinical case files utilizing client management software and follow all charting policies and procedures.

Maintain clinical metrics for each client including impact markers and end of service program evaluations.

Attend relevant staff and community meetings.

Network and develop reliable referral base from programs serving adults and their families.

Collaborate with all internal HSB program/departments.

Collaborate with community agencies who partner with Hospice of Santa Barbara.

WHAT YOU'LL NEED:

MSW or MFT

Bilingual skills (Spanish/English)

Demonstrated ability in care management and counseling related to end of life

Familiarity with local agency and community resources

Sensitivity and commitment to the hospice philosophy of care

Knowledge of Santa Barbara and Carpinteria medical, health care agencies and social service agencies

Experience in public speaking, effective presentation skills

Experience collaborating with volunteers

Bilingual skills useful, but not required (Spanish & English speaking/reading/writing)

Comfort working autonomously on cases

Ability to arrange own transportation; willingness to travel back and forth (SB and Carp)
