Qualifications:
High school Diploma Required, Associates of Arts or Bachelor Degree preferred.
Two years’ experience providing direct care management services to patients from diverse backgrounds in healthcare, or behavioral health setting.
Experience working as part of a multidisciplinary team.
Experience working with children/teens, families.
Knowledge of Tri-Valley/Bay Area health and social service resources.
Flexibility and creativity.
Strong organizational, administrative and problem-solving skills and ability to adapt, while maintaining a positive attitude.
Ability to prioritize tasks; work under pressure and complete assignments independently and in a timely manner.
Ability to effectively present information to others, including patients, Axis staff and community partners.
Ability to seek direction and approval as appropriate while also working independently and using professional judgment and diplomacy.
Understand concepts of integrated care services.
Knowledge of electronic health record and use of documentation to coordinate care.
Strong analytical, employee relations, and interpersonal skills.
Excellent writing, business communication, editing, and proofreading skills.
Ability to interact effectively and in a supportive manner with persons of all backgrounds.
Proactive, self-motivated and able to work independently as well as on a team with the ability to exercise sound independent judgment.

Must have good computer skills using Microsoft Office and the ability to use Axis departmental systems.

Essential Duties/Responsibilities:
Meet with patients presenting to the urgent care center and determine needs, provide resources and utilize urgent care team to provide services as needed.
Identify and collaborate with outside providers who will be providing longer-term care and ensure continuity of care as possible.
Complete documentation using the electronic health record in a timely fashion while adhering to related laws and regulations for documentation.
Attend weekly staff meetings.
Provide all services in accordance with accepted standards of care and Axis protocols within the parameters of agency contractual requirements.
Support the navigation of wrap-around preventative services for clients and families including enrollment in Medi-Cal, CalFresh, housing and other resources.
Encourage partnership between external entities such as schools, ACBH contracted services, California Children’s Services (CCS), and the Regional Center of the East Bay.
Tracking of patient referrals internally, externally and other reporting requirements mandated by funding source.
Participates in any other activities deemed appropriate by the IBH Director related to providing additional support to the urgent care team, and also the IBH team.
Work hand in hand with other care coordinators, and also care neighborhood professionals, in order to ensure all services are being provided in collaboration and are not redundant.
Support a positive work environment and foster teamwork.
Participate in staff meetings, attend other meeting and training events as assigned.
May be required to perform other related duties, responsibilities and special projects as assigned.

Physical Demands and Working Conditions:
Work is normally performed in a typical clinic office work environment (and, in some cases, telecommuting sites). The physical demands described here are representative of those that must be met by an employee to perform the essential functions of this job successfully. Physical: Occasionally required to carry/lift/push/pull/move up to 20lbs. Frequently required to perform moderately difficult manipulative tasks such as typing, writing, reaching over the shoulder, reaching over the head, reaching outward, sitting, walking on various surfaces, standing, and bending. Occasional travel to other Axis health centers and other occasional travel will be required.
Equipment: Frequently required to use repetitive motion of hands and feet to operate a computer keyboard, telephone, copier, and other office equipment for extended periods.
Sensory: Frequently required to read documents, written reports, and signage. Must be able to distinguish normal sounds with some background noise, as in answering the phone, interacting with staff etc. Must be able to speak clearly, understand normal communication, and be understood. Must be able to see clearly and have the ability to judge distances and spatial relationships to see objects where and as they actually are.
Cognitive: Must be able to analyze the information being received, count accurately, concentrate and focus on the given task, summarize the information being received, accurately interpret written data, synthesize information from multiple sources, write summaries as needed, interpret written or verbal instructions, and recognize social or professional behavioral cues.
Environmental Conditions: Frequent exposure to varied office (medical clinic/office) environments. Rare exposure to dust and loud noises.
