The Clinical Case Manager facilitates Child and Family Planning Meetings using wraparound principles. Santa Clara County Connection's goal is to help children and their families receive intensive treatment in the least restrictive environment that is appropriate. The Clinical Case Manager provides assessment of the client and family's needs and leads the team in providing treatment to support the family toward meeting their goals.

Seneca's Wraparound programs offer family-centered, individualized, culturally relevant, and strength-based support to families with complex and enduring needs. Wraparound works in close partnership with Department of Family and Children Services, Juvenile Probation, Behavioral Health, and group homes, providing strengths-based and outcome-oriented alternatives to group care placements for children and youth by drawing upon community and family strengths. Treatment and support services are tailored to address behaviors and meet family needs, even as they change over time.

Responsibilities:

Facilitation of Child and Family Planning Meetings. This includes: prepare for the meeting, coordinating attendance, facilitate the meeting, write meeting notes and distribute them within two days, write the mental health notes, and clean up after the meeting

Case management and coordination of care

Collaborate with county and community partners including but not limited to county social workers, probation officers, outside therapists, psychiatrists, teachers, etc.

Travel up to 90 mile service radius

Complete comprehensive assessments, treatment plan of care, mental health notes, Family Team Meeting notes, Wrap Action Plan, incident reports, and other documentation as necessary

Intervene with clients' behavior management programs and provide crisis management as needed. In rare circumstances when there is serious threat of harm, this may include physical management of the clients (implementation of Seneca Center's Ahimsa Model Training)

Hours:

Monday - Friday; flexible schedule required with ability to work early mornings, evenings, or weekend hours as needed

Willing to respond to crisis situations and be part of a rotating on-call team (24 hours approximately three times per month

Qualifications:

Bachelor's degree required and MHRS eligible

At least 4 years of experience in a mental health setting required

Own vehicle, California driver's license and insurance

TB test clearance, fingerprinting clearance, and any other licensing or certification requirements

Experience implementing family-focused wraparound services a plus

Experience working with children/youth and families!
