Position Details

ColumbiaCare provides Intensive Case Management Services (ICM) that are a high intensity flexible, community-based package of care that includes assessment, strengths-based treatment planning, and recovery oriented case management that helps identify the individual’s service needs, and establishes and maintains community support systems and service coordination.

About the Position

We are looking to hire a professional ICM Program Manager to work closely with Jackson Care Connect. This position provides both administrative and clinical supervision to a highly cohesive team of therapists and case managers who provide community based clinical services to severely and persistently mentally ill clients in multiple settings including hospitals, residential treatment facilities, in the individual’s home or other community setting. Services may continue on a short or longer-term basis with the purpose to stabilize and maintain the client safely in the community without hospitalization, and to transition clients to the most appropriate level of care. In addition, close collaboration with other ColumbiaCare Services Programs under the guidance of a team of Directors is an integral part of the success of the ICM Program.

Work Schedule: Monday - Friday; 8:30am-5:30pm

Leadership of the ICM program must be able to work closely with hospitals and other community partners, individuals, outpatient service providers and Jackson Care Connect to maintain the structure and integrity of the ICM Program. The ICM Program Manager must be able to calmly and professionally respond to an ever-changing environment and exercise excellent clinical and problem-solving skills. The best candidate for this position is an experienced clinician and leader who enjoys working in a highly demanding yet rewarding environment. A clinician/leader who has experience in crisis or crisis intervention and who can provide strong clinical guidance will be the best fit for this team. This position also requires successful completion of Asante’s credentialing requirements, including mandated vaccinations, drug testing, and Asante computer program training/competency demonstration.

Qualifications

MUST be QMHP Certified through Mental Health & Addictions Certification Board of Oregon (MHACBO) -OR- MEET eligibility qualifications to register with MHACBO

State of Oregon QMHP credentials:

MUST have a Master’s degree in Psychology, Counseling, Social Work or related field; Bachelor's degree in Nursing and be Licensed by the State of Oregon

MUST meet OAR requirement for QMHP by having completed TWO (2) YEARS of post-graduate clinical experience in a mental health treatment setting.

Additional Requirements:

MUST have TWO (2) or more years supervisory experience Professional level administrative experience within a multi-discplinary Human/Social delivery system.

Successful applicants MUST also have a valid Oregon driver’s license, maintain a safe driving record, and the ability to pass a DHS criminal background check.

Salary & Benefits

Starting at $5,000 per month/DOE. This is a salary exempt position.
