As we grow, we are now seeking a part time or full time Palliative Care Case Manager to join our team in the San Luis Obispo & SLO Coast area. The Nurse will be responsible for assessing patient needs, diagnosing new issues, actively managing and treating symptoms, including social, psychological and physical, as well as evaluating the plan for effectiveness. The regular work schedule is Monday-Friday 8AM-5PM with occasional weekends on call. NO Night Call!
