The Registered Nurse (RN) is an integral part of the patient care team. The RN assists in the delivery of high quality, individualized patient care. The RN autonomously practices within the scope of RN licensure as defined by the California Board of Registered Nursing, and the Nurse Practice Act. The RN advocates for patients and their families, the care team and the organization. The RN designs and carries out the plan of care utilizing the nursing process of assessment, diagnosing, planning, implementing and evaluation.Is aware of patient presenting problem, and past medical history. Completes head-to-toe assessment and gathers relevant patient data (vital signs, labs, etc.) to gain clear understanding of patient status. Monitors for improvement or deterioration of status and intervenes as appropriate. Notifies MD of changes in status. Accurately and safely administers medications per orders; scans 90% or more of all administered medications. Titrates infusions per organizational protocols. Assesses and documents patient response per policy. Functions as part of an interdisciplinary team by working collaboratively with physical therapy, case management, respiratory therapy, nutrition, pharmacy and the MD; attends interdisciplinary rounds and reports nursing specific knowledge and assessment data to the team. Advocates for the patient and family.Facilitates communication between disciplines, the patient and family; utilizes the patient white board as a communication tool and ensures it is complete and up to date at the start of each shift. Provides a hand-off that is concise and accurate, and utilizes an SBAR format. Ensures hand-offs include a bedside check for safety and patient belongings, chart check for new orders and results, and items needing follow-up Is familiar with organizational goals relating to patient 5 Star experience, and has a basic grasp of HCAHPS scores and Service Excellence. Models professional behaviorsSupports a safe and healing environment for each patient by keeping rooms neat and tidy, and free from clutter. Ensures all patient safety interventions are in place at all times - such as keeping the call light and patient belongings within reach of the patient; bed/chair alarms are on and functioning; and/or all other patient specific safety interventions are in place. Recognizes importance of customer service, is welcoming and friendly. Assists and guides visitors as needed, answers the phone with a professional manner and tone. Is sensitive to cultural differences and treats all people with respect and compassion. Maintains a professional personal appearance and manner at all times, in accordance with SVH policy.Supports a healthy working environment by volunteering discretionary effort constantly, is curious and not judgmental, maintains a positive attitude and fosters a spirit of teamwork among all disciplines. Seeks solutions and engages in problem-solving, avoiding gossip and negativity. Offers expert teaching and advice to all members of the team.Supports a safe working environment by keeping hallways and workstations free of equipment and clutter. Puts equipment and unused supplies away when no longer needed. Submits work orders for faulty or broken equipment to appropriate team (Biomed, Engineering, IT). Cleans and disinfects equipment per SVH Infection Prevention policy. Performs hand hygiene before and after every patient interaction and when hands are visibly soiled. Reports unanticipated events as appropriate via the Midas SystemHas basic understanding of the impact of healthcare reimbursement on revenue; practices financial stewardship by not overusing or wasting supplies; practices good time management to avoid incidental overtime; completes accurate documentation in a timely manner, particularly as it pertains to the discharge process, to ensure appropriate charges are captured as needed. Supports throughput efforts for timely transitions of careMaintains personal responsibility for keeping all employment requirements in good standing, including but not limited to California license to practice, BLS certification, annual employee health requirements and annual employee education requirements (i.e.Healthstreams, skills lab.) Education: Graduate of an accredited nursing program. BSN preferred.Experience: Minimum of 2 years of Acute Care experience, with additional experience in telemetryLicenses & Certifications: Current RN license by the State of California; Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) by an American Heart Association approved provider.Required Skills & Knowledge: Proficient computer skills, strong critical thinking and analytical skills, multi-tasking and time management skills.
