Job Description

The Therapy Field Supervisor is responsible for ensuring that patient rehabilitative care is coordinated and managed appropriately in the field. This position is responsible for ensuring that field therapy services are delivered appropriately and works laterally with the Rehabilitation Director.

1. Works in conjunction with Rehabilitation Director in providing support to therapy clinicians in the field. Instructs and guides therapists to promote more effective performance and delivery of quality home care services, and is available at all times during operating hours to assist therapists as appropriate.

2. Assists therapists in establishing immediate and long-term rehabilitation goals, in setting priorities, and in developing plan of care.

3. Assists therapists in overseeing assignment and supervision of therapy assistants.

4. Attends case conference meetings with organization personnel to facilitate coordination of care.

5. Participates in quarterly record reviews as needed by Rehabilitation Director and communicates findings and recommendations to Rehabilitation Director and appropriate organization personnel.

6. Completes on-site supervisory visits as necessary.

7. Assists in maintaining accountability for therapy staff productivity, utilization and documentation standards as needed by Rehabilitation Director.

8. Assists in the screening and interviewing process of new organization personnel and makes recommendations for employment of individuals. Assists in the orientation of new therapy personnel.

9. Assists Rehabilitation Director in the planning, implementation and evaluation of inservice and continuing education programs.

10. Assists, as needed, in the formulation, revision, implementation, and evaluation of policies and procedures, as well as strategic goals and objectives.

11. Complies with accepted professional standards and principles.

Qualifications

1. Current state license as a Physical or Occupational therapist or Speech/Language Pathologist.

2. Graduate of an accredited therapy program.

3. Two years management or supervisory experience in a rehabilitation therapy department or program, preferably home care.

4. Complies with accepted professional standards and practice.

5. Complies and maintains current CPR certification.

6. Licensed driver with automobile that is in good working order and insured in accordance with organization requirements.
7. Demonstrates excellent observation, verbal and written communication skills.
